Job summary

An exciting opportunity has arisen to join a team of pathway and system integration leaders as part of the ICB in Leeds. The teams purpose is to draw on a matrix of experts (including facilitation, programme management, Population Health Management and Quality Improvement), to support partners to integrate services, pathways, and resources around the needs of populations, improving flow and reducing duplication.

The main focus of the role will be the healthy adults population segment which in Leeds encompasses the prevention and communities of interest/tackling health inequalities agendas. The role will work within the planned care, cancer and healthy adults team and collaborate on programmes across a number of key focus areas both within and outside of this portfolio, as determined by system needs and prioritisation.

The post holder will manage a portfolio of services and pathways delegated by the Programme Directors in line with agreed priorities. They will lead and participate in matrix teams across the Leeds Health and Care Partnership to enable the improvement of services and outcomes for agreed populations. The post holder will develop and deliver programmes in support of the Partnership priorities.

Main duties of the job

To support the Partnership in improving the integration of services across providers and disciplines to improve patient experience

To support the Partnership in developing a more targeted approach which enables resources to be geared towards where they will have greatest impact on health outcomes

Lead and monitor transformational projects and programmes and drive transformational change, service improvement and innovation with the programmes to ensure these meet agreed milestones and intended outputs and outcomes, reporting progress through agreed governance structures.

Work effectively with clinical leaders to design and deliver key projects, including developing outline proposals and full business cases using highly complex information, regularly presenting the case for change to decision making groups, leading the discussion using negotiating and re-assurance skills and ensuring agreed outcomes.

With support from colleagues, analyse a wide range of highly complex data sources, identify gaps in service provision and develop strategies to tackle unmet need.

Prepare reports and deliver presentations on highly contentious health issues to a wide range of audiences

Working with partners, undertake best value reviews of services to ensure lean, efficient and cost effective services.

Job responsibilities

This is an important role for the Pathway and System Integration Business Unit, which focusses on tackling health inequalities and in particular leads on the programme delivery of the CORE20plus5 standards across our work plans, as well as coordinating several system wide groups such as the Healthy Adults Population Board and associated subgroups.

The post holder will provide expertise and leadership to the system both in regards of specific contracting and service provision in these areas, and in terms of strategy and direction, advising and reporting to senior leadership on direction of travel and progress and coordinating efforts to tackle health inequalities across multiple population segments.

The post holder will be confident in data analytics and will be capable of identifying and driving improvement based on both qualitative and quantitative data.

The post will be a key liaison point with the WY health inequalities programme and population health management team in this respect.

The post holder will have the skills and behaviours to develop strong relationships across a wide range of partners including the VCSE and Leeds City Council. They will have the resilience and tenacity to navigate these relationships in the current financial context, preserving and promoting partnership working whilst delivering financial sustainability and best value.

The post holder will be passionate about prevention, improving population health and particularly about improving the health of the poorest in Leeds the fastest. They will deliver their work portfolio with a dedicated focus on this direction of travel but also be open and flexible if required to work with other population groups or service areas in line with emerging priorities.

Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in a specialist area Evidence of continuing professional development

Significant prior experience of working within service redesign, pathway improvement or other commissioning functions Experience of leading and managing staff Experience of communicating and engaging effectively with external agencies/stakeholders Experience of using performance management/service improvement methodology Knowledge and experience of programme and/or project management

Desirable

Experience of working in the NHS or local authority (working cross-sector and cross-settings) Experience of working with clinicians/registered professionals Experience of co-production with service users or carers
