Job summary

This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust. This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays.

This is an exciting opportunity to gain experience working as a senior member of the Children's Community Nursing team with specific responsibility for planning, delivering and supervising care for children and young people with acute conditions, complex needs and disabilities, and their families, in their own homes, schools, clinics and other community settings.

Main duties of the job

You will work as part of the Children's Community Nursing Team with responsibility for managing a defined caseload of children. The post holder will assess, coordinate, deliver and supervise home-based packages of care for individual children on your caseload. This will be working within the child / young person's own home, school or other community based settings the child / young person may attend..

You should be flexible in your approach, have good time management and IT skills and be able to work on your own initiative as well as within a team. The post holder will be expected to develop excellent partnership working relationships in order to provide seamless services to Children and Young people.

Successful candidates should also have knowledge of Safeguarding policies and procedures. Having a degree of flexibility, being responsive to change, and having the ability to work in a creative way are also essential requirements for this post

Job description

Job responsibilities

The post holder will work as a senior member of the Childrens Community Nursing team with specific responsibility for planning, delivering and supervising care for children and young people with acute conditions, complex needs and disabilities, and their families, in their own homes, schools, clinics and other community settings.

The post holder will be expected to demonstrate high levels of autonomy and decision making skills and undertake systematic, comprehensive health assessments. The post holder will ensure that high standards of service/care are maintained and ensure that all services are responsive to the needs of children and their families. They will also useevery opportunity to promote health and well-being of children, recognising that parents need to be empowered to care for their post holder will teach and provide expert knowledge and advice to other Registered Nurse, Health Care Support Workers and Health Care professionals within the Trust, Specialist Services Directorate, and Primary Healthcare Teams in relation to clinical care and other related topics

First Level Registered Nurse or RSCN Educated to Degree level or equivalent knowledge and experience
