An exciting opportunity has arisen for an enthusiastic individual to join the Patient Flow team. You will operate in a non-clinical role and have senior administrative duties for a designated directorate / division / peripheral site. Reporting to the Matron – Patient Flow you will focus on the SAFER Patient Flow Bundle reducing internal delays and waste. You will be expected to achieve measures of success / key performance indicators set by your line manager.

You will be expected to have the ability to cope well under pressure, possess good customer care skills, be able to communicate at all levels, have good organisational skills and the ability to prioritise your own workload. As a PFF you will be responsible for booking transport and support the organisation to achieve discharge pathways.

Main duties of the job

The key duties of the role are to support the wards by coordinating the patient’s pathway of activity through the hospital. The role has a requirement to work closely with the ward teams and discharge lounge, as well as keeping them up-to-date with any changes in pathway plans and also gathering information to ensure activities are carried out in a timely manner.

You will need keyboard skills and an aptitude for using a variety of IT systems, together with a degree of flexibility with your working hours. You will be working as part of a team and play an important part in providing administrative support to clinical areas.

This is an exciting and challenging opportunity to work across the Divisions to spread and sustain the NHS Improvement “SAFER” Patient Flow Bundle and lead on the improvement work being undertaken to reduce longer lengths of stays within ELHT.

You will Prioritise time and use skills in the Discharge Process, from admission through to discharge to:- contribute to the patient’s care plan, remove any barriers to their managed discharge date, facilitate timely discharge, Complete the Criteria to Reside and Discharge Pathway for each patient. Provide specialist input to coordinated care and the senior review process at MDT board-rounds and attend Every day meeting with updates on estimated date of discharge and longer length of stay.

Detailed job description and main responsibilities
  • Relevant ward based experience in discharge planning
  • Evidence of NVQ level 3 , working towards an NVQ level 3 or equivalent experience
  • Experience of working across professional boundaries

This is a full time and part time position working on a 7 day rota covering the hours of 8am-8pm each day.
