We are looking for an experienced and passionate Mental Health Community Care Navigator to work as part of our Community Mental Health Team.

I will be integrated in a primary care setting and will work alongside mental health and primary care professionals to support people with personalised support. Specific to the role will be supporting mental health service users, with continuation of care and support to address the broader social determinants of health and wellbeing. I will ensure people are linked to appropriate community health, social care and voluntary sector services and support networks.; reducing the risk of social exclusion or mental health relapse. You will also direct people into specialist mental health services where appropriate and support people throughout their journey.

How you will make a difference

Reporting into the Primary Care Mental Health Liaison Practitioner, you will be fully integrated into a multi-disciplinary team working alongside mental health and primary care professionals to support service users with personalised support, and most importantly delivering great outcomes for people using services. You will proactively engage with service users, identify non-clinical needs and provide the relevant support and guidance required to improve the patient’s mental wellbeing and quality of life.
