Job summary

The Therapy team at Arthur Rank Hospice Charity providesspecialist palliative care to patients across our hospice services, includingour in-patient unit, Specialist Palliative Care team and the Living WellService. We have an opportunity for an Occupational Therapist to jointhis team in a full/time working hours . The working hours can be discussed during the interview.

Main duties of the job

The role will work closely with Physiotherapy colleagues toprovide care, focussing on fatigue management, anxiety management andbreathlessness management.

Successful candidates must be HCPC registered with a diploma/ degree in Occupational Therapy, along with knowledge of, and experience ofproviding, palliative care to patients.Due to the nature of the role its a requirement to hold a UK driving licenseand have access to a vehicle for work purposes.

Interviews for this role will be held on the 17thand 18th September.

Job responsibilities

You will be partof the Therapy Team which provides specialist palliative care to patientsacross the clinical areas of IPU, SPCHT, LWS.

Job Purpose:

Todeliver a high standard of Occupational Therapy services to patients referredfor Specialist Palliative Care. To planand deliver those services in partnership with multi-disciplinary teamcolleagues on the in-patient unit, outpatients, living well service and withpatients living in the community, to achieve/ maintain quality of liferegardless of diagnosis and prognosis. To maintain associated records inaccordance with HCPC and RCOT guidelines.

To actively participate in improving the serviceand in collaboration with the line manager, support future developments. Thesewill include involvement in clinical audit, education and research programmeswithin the organisation.

Main Responsibilities:

Clinical

To provide holistic OccupationalTherapy assessment and diagnosis using a rehabilitation model that includes thephysical, psychological, social, emotional and spiritual aspects of the personreferred for specialist palliative care, with diverse orcomplex needs and multiple pathologies, using professional clinical reasoningskills and evidence-based practice and to formulateappropriate treatment plans based on clinical reasoning

Toactively identify goals, plan suitable Occupational Therapy (OT) intervention,deliver and evaluate OT service provision, in partnership with patients,carers, multi-disciplinary team colleagues, and other professionals/ agencies,making appropriate referrals to others as required

To communicate effectively withpatients, families and carers especially where there are barriers tounderstanding, requiring empathy, reassurance and persuasive skills.Maintaining honesty and integrity in all situations.

Tocommunicate effectively with multidisciplinary team colleagues and otherprofessionals/ agencies in the provision of care for patients with complex,emotional/psychological, spiritual, and physical needs and refer to appropriatedisciplines. Attend relevant meetings as required.

Whereappropriate, contribute to the effective discharge of patients from theinpatient unit liaising with patient, family, colleagues, and members ofthe

primaryhealthcare team

Toarrange and carry out timely access and home visits.

To order appropriateequipment for safe discharge or to maintain safety in the patients home

Participatein teaching, assessing and evaluation of OT students to graduate level withinthe clinical area of specialist palliative care.

To actively contribute to the Educationprogramme provided by Specialist

Palliative Care Services

Workflexibly across service areas to meet service needs.

To act as a Moving and Handling link person for allclinical areas to facilitate Moving and Handling risk assessments and riskreduction measures.

Operational

To be professionally and legallyresponsible for all aspects of your own work,

including themanagement of patients in your care, and to respond appropriately to serviceneeds.

To always maintain appropriatestandards of confidentiality.

Toundertake appropriate delegated responsibilities from senior staff and linemanager

Tomaintain a high standard of clinical competence in order to assist and activelycontribute to the evaluation, development and ongoing audit of OT andmulti-disciplinary team services offered to patients within SpecialistPalliative Care

Toactively contribute to OT service development within Specialist Palliative CareServices

To positively promote OT to patients,carers, multi-disciplinary team colleagues, and other members of the public andprofessionals.

To be responsible for organising andplanning your own case load to meet

service and patient priorities

Todeputise for the Team Lead in their absence as required.

To be responsible for maintaining owncompetency to practice through CPD activities and to maintain aportfolio which reflects personal development. To participate in clinicalsupervision and in the staff appraisal scheme

To undertake delegated responsibilityfor the assessment and evaluation of junior staff appraisals.

To identify and address gaps inservice delivery and working practices within specialist palliative careservices, through independent observation or through joint working with others

Reportsany defect in equipment immediately to the person with overall responsibilityat that time.
