Are you a practitioner looking for a change in role or to further your experience or development working with children and young people, working in the Community?
We are looking for motivated professionals with good knowledge and experience of working with children, young people and families.
You should have the ability to work within a skill mixed team to deliver allocated components of the Healthy Child Programme 5-19 (25 for SEND), to develop positive relationships with children, young people and families which enable them to be engaged, participate, take responsibility and make informed choices and decisions.
Working as a team player under the delegation of a School Nurse your role will require you to work in partnership with schools, to promote and enhance the health and well-being of children and families, to deliver specialist targeted interventions and groups to meet the needs of children, young people and families identified as at risk of poor outcomes or with complex needs.
HCRG are working as an integrated partnership with Barnardos as The Essex Child and Family Wellbeing Service (ECFWS), delivering innovative and refreshing health and social care from pre-birth to 19 years services to Essex Families.
We have developed our own unique approach where our children & young people and families are at the centre of everything we do.
We are currently looking for practitioners to join our Healthy Family Teams to help us continue to improve things for the better.
Main duties of the job

Delivery of the healthy child programme 0-19 pathway at four levels as delegated by the Specialist Community Public Health School Nurse To plan and deliver engaging personal and social development sessions for young people aged 11-19 covering a range of topics To provide Brief Intervention and Solution focused support in relation to a range of issues including emotional well-being, exploitation, sexual health, relationships, substance and alcohol misuse, risk taking and anti-social or offending behaviour. To establish contact and build sustainable working relationships with young people and families at risk of social and educational exclusion, ensuring the personal and social education needs of young people are identified and met. To work in a multi-disciplinary framework with key partners, including the statutory and voluntary sector, in identifying and responding to young people and family needs. Delivery of targeted Health promotion campaigns, through mediums such as social media and PSHE Work with parents, carers and families to ensure that they are appropriately engaged in the support process and that they know how to access support for themselves and their children.

Ability to identify, assess and manage risk and develop interventions to improve health and wellbeing.
High level of understanding of the educational, employment and social care needs of children and young people
Excellent verbal and written communication skills, with the ability to communicate complex issues in plain language at all levels both within and outside the organisation.
Experience of managing a caseload of young people, from vulnerable groups, and the ability to differentiate clearly between personal and professional roles
Ability to develop and maintain good working relationships with partner agencies colleagues, external bodies and service users
Able to demonstrate a commitment and ability to promote and safeguard the welfare of children and young people with the skills to build sustainable professional boundaries and appropriate relationships with young people
Demonstrable understanding of: Childrens Act, Healthy Child Programme Choosing Health and other relevant national and local documents
You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages Outlook, Word, Excel.
You will have a valid UK driving licence and have access to a car for work use
