An exciting opportunity has arisen for an experienced Mental Health Nurse to join the Intensive Outreach Team.
We welcome applications from enthusiastic, self motivated, and recovery focused practitioners who value working in a supportive team environment. Experience in psychosocial interventions, family work and/or substance misuse would be advantageous.
You will be working offering recovery focused, intensive support to a caseload of people with complex mental health difficulties, who experience psychosis and who struggle to maintain engagement with community services, and their families, using a whole team approach.
The role will involve working with the CMHT's and EIT across north Staffordshire coordinating the care of individuals in line with changes in the national direction of the Care Plan Approach offering evidence based interventions to address core needs as well as manage risk in a community setting.
Close working relationships with inpatient, recovery and rehabilitation, other statutory and third sector agencies will be necessary to support people with multiple needs and who often have chaotic and disparate engagement with services. We strive to think creatively to ensure the highest quality care and are committed to continuous improvement ad adapting the service to the needs of the population. Your input to the development of the service is a valued part of the role.
Main duties of the job
To work as an integral part of a multi-professional team working with individuals with severe and enduring mental health problems who have problems in engaging and maintaining lasting and meaningful contact with mental health services. The nature of the client group and the philosophy of the team will require the post holder to work flexible hours.
The post holder will be responsible for the assessment of care needs, the development, implementation and evaluation of recovery based care programmes and the setting of standards of care for this complex group.
To undertake an assertive outreach approach to engage the identified individuals and to provide support and interventions that aim to meet their needs.
The post holder will deputise for the Caseload Manager when required.

Job responsibilities
To have the responsibility for the delivery of an Assertive Outreach/Intensive Support approach/intervention to specific individuals.
Carry out all relevant forms of care without direct supervision.
Coordinating clinical activity, liaison with the rest of the Community Mental Health Team (CMHT) and other appropriate teams, services and agencies.
Immediate problem solving and decision making and contributing to long term planning for future developments both within the immediate clinical area and within the service area.
To ensure that the values and principles of the organisation and of the service area are translated into the philosophy and operational policy of the clinical team and that these are evident in everyday practice.
