To have the responsibility for the delivery of an Assertive Outreach/Intensive Support approach/intervention to specific individuals. Carry out all relevant forms of care without direct supervision. Coordinating clinical activity, liaison with the rest of the Community Mental Health Team (CMHT) and other appropriate teams, services and agencies. Immediate problem solving and decision making and contributing to long term planning for future developments both within the immediate clinical area and within the service area.

To ensure that the values and principles of the organisation and of the service area are translated into the philosophy and operational policy of the clinical team and that these are evident in everyday practice.
