Job summary

Felixstowe Community Healthcare Team has an opening for a fixed term/secondment part-time ( hours per week) B5 Community Nurse.

The Felixstowe Community Healthcare service is part of a wider Integrated Neighbourhood Team (INT) consisting of district & community nurses, generic workers, community matron, occupational therapists, physiotherapists and Social Care Workers led by a B7 registered health team lead and overall managed by an INT manager.

The team is busy, dynamic and proud of their holistic care approach. The team continues to change to support developments in NHS which includes close working relationships with the local statutory, non-statutory and voluntary services including GP, acute hospital colleagues, GP Federation,Social Prescribers, SuffolkMind, NSFT and many local community voluntary groups.

You will be actively supported by regular supervision and individual annual appraisal. You will be expected to work autonomously and on behalf of the patients served by the local area. You will have access to skills development training relevant to this post.

The role involves providing nursing interventions to the housebound residents of Felixstowe and surrounding area, who are 18+ and registered with a Felixstowe GP. Our main aim is to support, maintain and encourage people to make healthy lifestyle choices in the ageing well process.

Main duties of the job

Triage, assess, plan, implement and evaluate all aspects of nursing patient care, and develop care plans which may need to be delivered from a range of options. Ensure informed consent is obtained and documented prior to initiating interventions. Following a holistic assessment, devise an individualised, evidence-based plan of care for each patient, modifying it as required. Assess patients' holistic needs, communicating complex and sensitive information to patients and carers. Use tactful and persuasive skills to gain the patient's co-operation in their care management plan. Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated. Maintain skills with regards to latest technology Participate in multi-disciplinary/multi-agency meetings as appropriate, Gold Standard Framework. Ensure own workload is planned and prioritised according to patient needs. Ensure patient held records are completed for each visit, and that SystmOne inputting is completed daily. Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advanced sensory skills, manual and mobilising skills.

Conditions include long-term neurological disorders, palliative and end of life care; diabetes; continence; acute medical episodes, wound care and phlebotomy all with a view to preventing admission and supporting early discharge. Many of our patients have a complexity of needs across the spectrum of physical, mental and social care health.

Current NMC registration RN Qualification (Part 1 of register) or equivalent. Car Driver and have access to a car for work which is community based

Leg Ulcer Management IV Administration / cannulation ENB 998/mentorship preparation or equivalent Mentorship qualification is essential for the post, but not essential at interview. Previous experience working in the community.
