Job summary

We are looking for Highly Specialist Community Physiotherapist to join our Integrated Community Rehabilitation Teams covering the West Dorset Cluster.

Our multi-disciplinary teams work with adult patients in their own homes, Community Hospital Wards and Care homes to promote independence and improve quality of life by supporting them to return to and remain at home. The role of Highly Specialist Community Physiotherapist provides highly specialist assessment, advice and interventions to avoid unnecessary admissions to hospital, support timely discharges from the Hospital, and provide Rehabilitation programmes for patients in a Community setting.

You will be required to provide holistic clinical assessment of patients and be able to problem solve and make decisions about their treatment, goals and discharge plans. The role will also require the post holder to support more junior staff with complex case management and clinical supervision where required.

We offer opportunities for further training and development. There is also the chance to be involved in Service development projects.

Employment in this post requires an Enhanced DBS check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Main duties of the job

To undertake highly specialist autonomous assessments, interventions, and management of patients on an independent caseload and as part of the wider multidisciplinary team.

Provide highly specialist advice to others regarding the management and care of patients / service users.

Responding to patients in crisis in their own homes to avoid unnecessary hospital admission.

Providing follow-up and monitoring through clinical assessment and interventions to support people to return and remain at home.

Undertake clinical observations and holistic assessments, acting on findings and liaising with the MDT as required.

Devise highly specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.

Provide a high quality person centred approach to care delivery which always considers people's safety, privacy and dignity.

Provide highly specialist programmes of rehabilitation to be delivered to patients in either their own home, community hospital, or care home setting.

Person Specification

Knowledge, Skills and Training

Essential

Registered practitioner to degree/diploma level supplemented by Masters level study or working towards in a relevant subject or demonstrable extensive experience in the relevant specialty Learning and Assessing in Practice Qualification or equivalent practice assessors training Evidence of recent professional development in an up to date portfolio Membership of the relevant Professional Body Evidence of Leadership training and application in practice or willingness to complete Broad clinical and multi-agency knowledge

Job Specific Experience

Essential

Proven experience at Registered Practitioner Band 6 level in the community Able to demonstrate specialist expertise and knowledge underpinned by theory acquired through CPD in relation to clinical assessment, treatment and evaluation. Evidence of proactive involvement in the supervision and development of members of the multi-disciplinary team Evidence of involvement in the development of programmes of care, protocols and audit.

Personal Qualities/ Attributes

Essential

Able to communicate effectively at different levels of the organisation and with staff, patient/service users, visitors or external organisations both verbally and in writing in the exchange of highly complex, sensitive or contentious information which may require the use of negotiating and/or persuasive skills. Able to analyse and assess situations and to interpret potentially conflicting situations and determine appropriate action, where there is a range of options and judgement is required. Experience of planning and organising complex activities such as organising own time and that of junior staff and learners, planning off duty rotas, undertaking discharge planning involving co-ordination with other agencies and planning for sudden unforeseen circumstances. Ability to evaluate care leading to improvement in quality standards and service improvement Ability to use own initiative within sphere of authority
