The Clinical Nurse Specialist (CNS) will demonstrate a high level of practice in both direct patient care and in the ability to plan and develop services to support cancer care. Through the key skills of leadership, innovation and clinical expertise the CNS will facilitate and deliver patient centred care, which is efficient and effective.
The CNS is a core member within the multidisciplinary team (MDT). They will work collaboratively with clinicians and service managers to promote, develop, review and redesign services that are responsive to patient’s needs in line with national and local objectives. The CNS is an autonomous practitioner and has the freedom to act to influence patient care and
service provision.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the jobDemonstrate leadership through clinical expertise delivering high standards of personalised care by acting as a key worker, identifying needs through holistic needs assessment and co-creating a care and support plan with people living with cancer.
To undertake holistic needs assessment, care planning and evaluation from diagnosis, through treatment and beyond. This will necessitate high levels of autonomy and advocacy ensuring the delivery of ongoing appropriate care to ensure individuals have a good quality of life.
Lead on the development, implementation and evaluation of Nurse Led Clinics to support the delivery of Holistic Needs assessment either by telephone or face to face.
To establish working relationships and communication links between primary secondary tertiary and independent care providers to improve care for patients and provide specialist clinical expertise, advice and support when required.
Ensure that professionals adhere to the implementation and monitoring of relevant clinical guidelines, policies and procedures within their scope of practice and speciality.
To be the patient advocate within the site-specific Multi-disciplinary Team (MDT). To request appropriate clinical investigations to support patient’s assessment as deemed appropriate by their line manager.
