Job summary

We are excited to offer this opportunity for experienced and newly qualified, health visitors to join our unique and innovative health visiting service. Health Visitors in Portsmouth lead in the delivery of the Healthy Child Programme (HCP) alongside offering targeted early help and enhanced interventions (ECHO) to improve long-term outcomes for the most vulnerable children and families in the City.

The successful candidates will be proactive and enthusiastic individuals who are looking to further develop their public health nursing skills to deliver a quality evidence-based service to families. Applicants will be inspirational and motivational leaders, highly skilled in communication, with a strong approach to teamwork.

We are an integrated 0-19 service with the Local Authority and work with the wider multi-agency and multi-disciplinary childrens workforce to identify need and provide timely intervention at the earliest opportunity. Health visitors are well supported to achieve this through a developing Team Around the Worker model, utilising consultation with other specialist practitioners working across the 0-19 service in areas such as perinatal infant mental health, substance misuse, domestic abuse and children with disabilities.

Main duties of the job

Planning and ensuring the delivery of appropriate interventions, working in partnership with clients, other health professionals using the Team Around the Worker model. To act as a clinical leader and health expert ensuring evidence-based practice is delivered to improve and assure the quality of care and to shape responsive services to meet identified need. To provide high quality, safe and effective care to children and families through effective assessment, packages of care, interventions and on-going evaluation. To develop and maintain key working relationships with a wide range of professionals within Health, Childrens Social Care, Education, Children Family Hubs and other Statutory and Voluntary Agencies to ensure effective working to promote the health needs of children and families. Accountable for the delivery of the Healthy Child Programme within a designated caseload, working with SystmOne as clinical record and work planner. To identify and provide targeted services to vulnerable children and families based on the Early Help Assessment (EHA). To identify and assess children 0 to 19 years with special needs, offer professional support and coordination of health needs, liaising with the Children with Complex Needs Health Visiting Team (0-5 year) and other appropriate services.

About us

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by July 2024.Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on 1st July 2024. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.

Qualifications

Essential

Registered Nurse (Adult/Child/MH) or Registered MW and Registered Health Visitor SCPHN or equivalent on part 3 of the NMC register Teaching or Mentorship qualification Evidence of recent continuing professional development.

Experience

Essential

Working with children and parents in a variety of settings. Experience of effective working collaboratively with professionals from other agencies and disciplines Experience of designing and delivering parent education in groups and for individuals Planning and delivering health education information Evaluation of programmes of work. Experience of clinical audit Experience of involving children and families/carers in service development and care planning Experience and good understanding of clinical supervision, restorative and reflective practice
