We are a Reconnect Navigator, to enhance the quality and effectiveness of our recently established Liaison and Diversion - Reconnect Team.

What will be expected from me?

The aim of the Reconnect Service is to increase access and uptake of healthcare or relevant support services for individuals on release from prison, ensuring a safe transition from prison healthcare to community healthcare and other support services.

What I might be doing?

Engage with individuals prior to release from prison and following release, supporting their transition back into the community. Provide intensive support to individuals to overcome barriers to accessing local health and community-based services and to support their attendance. Facilitate ‘meet at the gates’ with prison leavers when needed and support them to attend all initial appointments as required Be responsible for a caseload of people with complex needs, providing high quality support.

Essential requirements:

The successful candidate will need to have:

An understanding of the issues faced by prisoners being released, such as substance misuse, mental health, accommodation etc, together with knowledge on how these issues can successfully be addressed Experience of managing a caseload of clients with complex vulnerabilities Experience and skills to work effectively with partner organisations in both statutory and voluntary sectors. A flexible approach to working is required as the post holder will be required to work weekends on a planned basis. The post holder will also be required to work outside normal office hours. The successful candidate will be subject to the prison vetting procedure prior to commencement in role.

What can I expect?

This is a full-Time post of 37 hours per week. 28 days of Holidays plus 8 Bank Holidays. Support and ongoing training.
