Key Responsibilities- Engage with individuals prior to release from prison and following release, supporting their transition back into the community.
- Provide intensive support to individuals to overcome barriers to accessing local health and community-based services and to support their attendance.
- Facilitate 'meet at the gates' with prison leavers when needed and support them to attend all initial appointments as required.
- Be responsible for a caseload of people with complex needs, providing high quality support.
- An understanding of the issues faced by prisoners being released, such as substance misuse, mental health, accommodation etc, together with knowledge on how these issues can successfully be addressed.
- Experience of managing a caseload of clients with complex vulnerabilities.
- Experience and skills to work effectively with partner organisations in both statutory and voluntary sectors.
- A flexible approach to working is required as the post holder will be required to work weekends on a planned basis.
- The post holder will also be required to work outside normal office hours.
- This is a full-Time post of 37 hours per week.
