We are seeking a skilled Reconnect Navigator to join our team at Wakefield Council.

About the Role

The successful candidate will be responsible for supporting individuals on release from prison, ensuring a smooth transition back into the community. This will involve engaging with individuals prior to release, providing intensive support to overcome barriers to accessing local health and community-based services, and facilitating 'meet at the gates' with prison leavers when needed.

Key Responsibilities

  • Manage a caseload of individuals with complex needs, providing high-quality support and guidance.
  • Work effectively with partner organisations in both statutory and voluntary sectors to ensure seamless service delivery.
  • Develop and maintain relationships with local health and community-based services to ensure access to relevant support.
  • Collaborate with colleagues to ensure effective communication and coordination of services.

Requirements

  • Proven experience of working with vulnerable individuals, particularly those with complex needs.
  • Strong understanding of the issues faced by prisoners on release, including substance misuse, mental health, and accommodation.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals and organisations.
  • Flexibility and ability to work outside normal office hours, including weekends.
