Main duties of the job
As a valued member of the Integrated Nursing team,you will join a uniqueand vibrant team providing symptom management, palliative and end-of-life care,as well as planned and emergency short breaks both at the hospice and in thecommunity. As a Haven House HCSW you will support the wider teamto ensure high standards of holistic care are delivered.
In return, we will support your wellbeing and offeryou a learning environment to develop your skills and competencies inpaediatric palliative care, supported by our Nurse Practice Educator.

Job responsibilities
JOB SUMMARY
Thepost holder will work as a skilled member of an integrated care team providing holistic care and support for individual children and young people with life limiting conditions and their families in the hospice, home and hospital as required.
Thepost holder will work in partnership with the child or young person and their family as well as other services providing support.
The post holder will be required to work a variety of shifts across 24 hours as the hospice provides care services 24 hours a day, 365 days a year.
KEYRESPONSIBILITIES
Clinical
Ability to prioritise and innovate when working in partnership with babies, children young people and families, demonstrating sensitivity to their individual needs by promoting play, inclusion comfort, dignity, privacy and quality of life.
Undertake duties relating to the care and personal care needs of the baby, child or young person. This will include intimate care, feeding play and general activities.
Provide a high standard of personalise holistic home care to babies, children and young people with life limiting conditions.
Ensure the safety of the baby, child or young person by monitoring and reporting all concerns and any changes to parents and the Integrated Nursing Service Lead as necessary.
Under the supervision of a senior member of staff provide a high standard of helathcare, always in partnership with the family.
Adhere to personalised plan of care for each baby, child or young person and Haven House's policies.
Promote and sustain a suitable environment in which the baby, child or young person feels safe and comfortable, being aware of individual physical, social, emotional and spiritual needs.
General
To ensure that holistic care is family centred, research based and meets all statutory requirements.
To be responsible for exercising teamwork skills, working collaboratively with all members of the Haven House Care Team and other services supporting the family.
To maintain an awareness of the multi-cultural nature of the Haven House catchment area, embracing diversity and promoting wider access for children, young people and their families.
To act in accordance with Haven House policies, guidelines and standards. To keep up to date wit national policies and guidance
Tobe responsible for further developing own knowledge and clinical skills andmaintaining own competency.
Toactively participate in reflective practice, mandatory training, appraisal managementand practice supervision.
Toshare information with colleagues at team meetings, when handing over care andat other informal contacts.
Toensure that the views of children young people and families are brought to theattention of senior care staff for consideration and to inform servicedevelopments.
Toensure that complaints are dealt with in a timely manner in accordance withhospice policy.
To maintain accurate records of all care provided. Toact as advocate for the child whenappropriate, maintain confidentiality.
ToMaintain clear and identified professional boundaries at all times
ToReport any equipment, maintenance or safety issues to the Facilities team andnurse in charge, ensuring that all Haven House property and equipment is welllooked after and maintained.
