Job summary

We have an exciting opportunity for a compassionate and motivated individual to join our King's Lynn Community Nursing Team on a permanent contract for either full and / or part time hours.

The friendly, supportive, and busy team support patients in their place of residence with a variety of interventions, including but not limited to the delivery of compassionate and empathetic palliative / end of life care, wound care, and diabetes management.

The service operates between 08:00 and 20:00 hours, seven days a week, and you will be required to work shifts between these times. We are willing to explore candidates who would like to consider applying for flexible working, part time, full time, and set shift patterns.

Main duties of the job

Assess, prescribe and delivery care for patients residing in the community setting to support self-management and improve quality of life. Provide education, support and prescribing planned care for individuals and their carers/family with long term conditions, such as diabetes and wound management. Support patients at the most sensitive periods of their life, including palliative care and syringe driver management. Work as part of a multidisciplinary team, linking with the local Primary Care Network, wider community stakeholders and your immediate team which consists of Community Nurses, Community Matrons, Community Nurses, Physiotherapists, Occupational Therapists, Assistant Practitioners and Healthcare Assistants. You will be supported to maintain competencies and training in specialist skills, such as bowel care, leg ulcer management, catheters, venepuncture, IM injections etc.

The right candidate will be supported to undergo a supernumerary period, with planned mandatory training, additional clinical skills, plus preceptorship (if applicable) through our Clinical Education Team. You will be supported in your role by Senior Band 6 Nurses and an experienced Clinical Lead Nurse and Clinical Operations Manager.

Job responsibilities

Assess, plan, implement and evaluate all aspects of patient care, and develop care plans, using clinical reasoning skills, that may need to be delivered from a range of options. Following a holistic assessment, devise an individualised, evidence-based care programme for each patient, modifying it as required. Assess patients holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan. Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance. Be required to use tact and persuasive skills in order to gain the patients co-operation in their care management plan. Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately. Be aware of assistive technology and utilise where appropriate. To participate in multi-disciplinary/multi-agency meetings as appropriate, Gold Standard Framework. Ensure own caseload is planned and prioritised according to service targets and patient need. Ensure patient held records are completed for each visit, and that SystmOne inputting is completed on a daily basis. Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advanced sensory skills, manual and mobilising skills. Contribute to the requisitioning of supplies and equipment through electronic ordering systems. Ensure informed consent is obtained prior to initiating interventions. Will be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies.
