You will work within the Crisis Resolution Home Treatment Team in Eastbourne, East Sussex, although the post will be working alongside service users within the High Weald, Lewes and Havens rural area. The role will support and enhance service delivery from qualified practitioners working with service users as an alternative to hospital admission.

You will proactively engage clients under the guidance of qualified practitioners within this rural area.

CRHT is a dynamic and enthusiastic team whose aim is to prevent hospital admission whilst supporting and treating service users undergoing a mental health crisis. The team is highly skilled, regular support and supervision will be offered to the successful candidate

We are building a new hospital in Bexhill to replace our inpatient services at the Department of Psychiatry. This means that, if you are successful in your application, you will be asked to transfer to the new facilities when they open in 2025.

Main duties of the job

The purpose of this post is to work as a member of a multi-disciplinary Crisis team which focus aims to prevent hospital admissions and to facilitate supported discharges from hospital.

The post holder with have specific responsibilities to of professionals and working across undertake a range of delegated clinical duties in a community mental health setting. The role will include evaluation and communication of clinical information with a range teams and the Trust to ensure patient centred care is provided at all times.

The post holder will contribute to the growth and maintenance of key functions of the clinical area / Crisis Team by carrying out assigned tasks, involving direct and indirect care to service users under minimal supervision and as directed by the Crisis Team Shift Coordinator, qualified staff and/or Team Leader. The role will include assisting in reviews of service users in the community; contributing to the assessment of their needs and ensure care plans are fully implemented. Personal development planning and targeted training will support the need to work autonomously, as a competent practitioner.
