This role involves working within a Crisis Intervention Team, providing essential support to individuals experiencing mental health challenges. The position focuses on delivering care that serves as an alternative to inpatient treatment, ensuring that clients receive the necessary assistance in a community setting.

You will actively engage with clients under the supervision of qualified professionals, contributing to their recovery and well-being.

The Crisis Intervention Team is a vibrant and committed group whose primary goal is to avert hospital admissions while offering support to individuals in mental health crises. The team is composed of skilled professionals, and the successful candidate will receive regular guidance and supervision.

Key Responsibilities

The primary objective of this role is to collaborate as part of a multi-disciplinary team focused on preventing hospital admissions and facilitating safe discharges from healthcare facilities. The incumbent will undertake a variety of delegated clinical responsibilities within a community mental health framework. This includes the evaluation and communication of clinical information across various teams to ensure that patient-centered care is consistently delivered. The role will involve assisting in the assessment of service users' needs, contributing to the development and implementation of care plans, and participating in community reviews. Personal development and targeted training will be provided to support the ability to work independently as a competent practitioner.
