Job summary
An exciting opportunity has arisen to join the nurse led integrated community respiratory team delivering care to the adult population of South East Essex.
The team have two sites, Rochford Hospital and Thundersley clinic and are looking for a practitioner (nurse or physio) with respiratory experience to support in delivering respiratory care to patients in a clinic setting or their own home.
The team consists of specialist nurses and physiotherapist, associate practitioners, HCA's and admin support. As an integrated service the team provide assessment and review of patients with a confirmed respiratory diagnosis including COPD, ILD and bronchiectasis, diagnostic spirometry, oxygen as the local HOSAR and respiratory physiotherapy.
The successful applicant will ideally have community experience which would be an advantage alongside respiratory experience, home exercise programme, oxygen assessment and will also participate in the delivery of the respiratory virtual ward. Demonstration of a good understanding in long term conditions, multi morbidities and the delivery of high level of respiratory care is essential alongside excellent communication and listening skills.
The successful candidate will demonstrate an ability to work autonomously as well as part of a team and have an understanding of primary care, secondary care, MDT working and ICB's.
Main duties of the job

Assist in the development needs of junior team members within the service. To contribute to the development of evidence-based practice using audit and other tools.
