Job summary

West Kent Primary Care (WKPC) is seeking to employ an Advanced Nurse Practitioner (ANP) on behalf of Weald Primary Care Network (PCN) to work across their member ANP will be responsible for managing and providing high-quality care for patients including residents within the local care homes.

Main duties of the job

Weald Primary Care Network (PCN) is looking for a caring,compassionate and experienced Advanced Nurse Practitioner (ANP) to join ourfriendly PCN team. The ANP will be responsible for managing and providing high-qualitycare for patients including residents within the local care homes. The post holder will apply advancedassessment, critical thinking skills and decision making when treating patients.This will include providing care from initial history taking, clinical assessment,diagnosis, treatment, to a treatment plan. They will work within their professional boundaries and scope of practice within theprimary care setting.

The Advanced Nurse Practitioner willprioritise and triage the needs of patients making any necessary referrals forinvestigations in line with local policies.The post holder will be required to communicate and work collaboratively with allstakeholders to meet the needs of patients, supporting the delivery of policyand procedures, and providing nurse leadership as required. They will focuson the four pillars of advanced practice: Clinical Practice, Leadership, Education,and Research.

Job description

Job responsibilities

Conduct weekly care home round to one care home within thePCN under supervision and support of GP attached to the home. Undertake new patient checks for all care home patientsreviewing Respect forms, deciding on treatment plans in discussion with carehome GP Undertake chronic disease management of care home patientsin consultation with the care home GP. Support admission reduction by reviewing those residentsdeemed at high risk of weekend admission to hospital. Work as part of a multi-disciplinary team to deliver safeand high-quality care. Carries out clinical nursing practice at an advanced levelusing expert clinical skills, critical judgements & knowledge to deliverappropriate and safe care. Assess, diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with acute, long termand complex health needs. Triages and treats patients, performing assessment of theirneeds and makes referrals as necessary to other members of the primary andsecondary health care teams as appropriate. Interprets investigation findings to determine effectivetreatment plans & discusses with GPs where a second opinion is required Proactively identify, diagnose and manage treatment plansfor patients at risk of developing a long-term condition, using bothnon-pharmacological and pharmacological treatments. Co-ordinate the planning and delivery of care ensuringpatients and carers or relatives are involved where required. Prescribing and reviewing medication that is appropriate forpatient needs and in accordance with latest evidence-based practice, nationaland practice protocols and within the scope of qualified practice. Diagnose and manage acute and chronic conditions, includingdrug and lifestyle treatment methods into a management plan. Request diagnostic investigations and interpret withinagreed protocols x-ray, blood tests, scans, etc. Assess, diagnose and treat patients in practice premises, theirown home or care home who require medical attention and/or refer to appropriateagency. Refers patients to other members of the multi-disciplinarypractice teams, secondary care organisations and other organisations asnecessary. Delivery of care in line with NSF, NICE guidelines andevidence-based care Maintains confidentiality of any information concerningpatients in accordance with current policy on informationgovernance. Demonstrates respect for privacy and confidentiality in allinteractions with patients and the public
