Job summary
As a new member of our nursing team you will be undertaking a widerange of nursingassessments and care interventions in order to meet the needs of our growing practice population. Obviously if working in primary care is something that is new to you, we will fully support you to grow and develop the skills and competencies needed for the role.
You will be a member of a highly experienced nursing team that consists of two Advanced Nurse Practitioners, a Trainee Advanced Nurse Practitioners, a Practice Nurse, two Health Care Assistants and a Phlebotomist.

Main duties of the job
Working as part of the multidisciplinary team to ensure high quality and consistent patient outcomes and experience for our patients, your day to day activities will include
Management of chronic diseases within own scope of practice, with a strong focus on patient empowerment Working with patients to develop and agree individual management plans Obtaining information using a lifestyle questioning approach in order to identify (and address) health promotion issues Obtaining pathological samples, including venepuncture Undertaking 12 lead ECGs Undertaking cervical screening Undertaking wound assessments, care planning and interventions Childhood and other routine vaccinations, including providing information to parents in order to improve uptake Participating in practice meetings and CPD activities relevant to the role

Parkside Medical Practice is located in the purpose built Riverside Building near the centre of Bulwell, with easy access to on site parking, bus and tram stops and Bulwell railway station.

Job Purpose
The Practice Nurse will undertake a range of nursingassessments and care interventions in order to meet the nursing needs of thepractice population; this will be done in collaboration with other members ofthe practice team and with other partner organisations as appropriate. Careprovided will remain evidence based in line with practice, local and nationalguidelines, policies and procedures as appropriate.
The Practice nurse will strive to secure the best possibleoutcomes for the patients under their care, whilst at the same time having dueregard for the financial, ethical and organisational impacts of their activity.
Clinical Practice
Working collaboratively with the wider practice team, helpto assess, deliver and evaluate nursing care in a holistic manner to thepractice population, including
Management of chronic disease from diagnosis,monitoring and development of individual care plans, agreeing where appropriatewith the patient and other health care professionals.
Provide information and advice on prescribed orover-the-counter medication on medication regimens, side-effects andinteractions.
Obtaining appropriate information using alifestyle questioning approach and identifying health promotional issues.
Working with patients to develop a managementplan where potential/actual health problems are identified.
Undertake the collection of pathologicalspecimens including intravenous blood samples, swabs etc.
Assessing and managing routine wounds includingtrauma.
Support and advise women requesting informationrelating to family planning needs.
Support and manage health needs of womenpresenting for cervical cytology consultations.
Implement and participate in vaccination andimmunisation programmes for both adults and children.
Removing and inserting pessaries
Undertake and interpret spirometry to enable thediagnosis and monitoring of Asthma/COPD .
Advice, support and where appropriate,administer vaccinations for patients travelling abroad.
Provision of ear care as appropriate, includingsyringing.
Assisting in providing minor-surgery sessionswhen required.
To carry out appropriate domiciliary visits topatients as directed by the Partners.
Communication and confidentiality
Utilise and demonstrate sensitive communicationstyles, to ensure patients are fully informed and consent to treat.
Communicate effectively with patients andcarers, recognising the need for alternative methods of communication.
Communicate effectively with colleagues; bothwithin the practice and within the wider health and social care environment.
Recognise that information obtained and accessedthrough the role of Practice Nurse will often be confidential in nature andthat the trust of patients and the wider community in the practice and thenursing profession may be seriously damaged through breach of this trust.
Maintain patient confidentiality; whilst at thesame time recognising the situations where confidentiality may lawfully bedisregarded.
Hold information about the management andworking of the practice as confidential, unless specifically authorised todisclose such information. This does not affect your rights and obligationsunder English Law.
Administration
Ensure accurate and timely notes of allconsultations and treatments are recorded in the patients notes accurately.
Attend and participate in practice and clinicalmeetings. The only reason for not attending will be annual/study or sick leave.
Assisting in stocking, cleaning, auditing andpreparation of clinical areas as may be required.
Training and Development
Actively participate in the annual appraisalprocess, through which training and development needs may be identified in linewith the requirements of the practice.
Remain up to date with developments within theirscope of professional practice; taking into account changes in practice, localand national guidelines as appropriate.
Attendance at courses and study days as agreedas appropriate.
Maintain compliance with all elements ofstatutory and mandatory training as notified from time to time.
Undertake the supervision, training, assessmentand support of unregistered colleagues and / or students as may be requiredfrom time to time, within the scope of ones own competence.
Professional practice
The Practice Nurse shall at all times
Recognise and work within their own competenceand professional code of conduct as regulated by the Nursing and MidwiferyCouncil (NMC).
Priorities, organise and manage own workload ina manner that maintains and promotes quality.
Engage in and operate the child and adultprotection policies and procedures of the practice and local authority asappropriate to their role; ensuring that safeguarding is prioritised, includingthe escalation of concerns in a timely manner.
Comply with the policies and procedures of thepractice; with particular regard to domestic violence, vulnerable adults,substance abuse and addictive behaviour and refer as appropriate.
Health and Safety
Comply with all policies procedures andinstructions relating to Health and Safety in the Workplace.
Monitor work areas and practices to ensure theyare safe and free from hazards and conform to health, safety and securitylegislation, policies, procedures and guidelines.
Maintain an awareness of national standards ofinfection control and cleanliness and Regulatory / contractual / professionalrequirements and good practice guidelines.
Ensure safe management of sharps storage anddisposal.
Recognise that there may be times where loneworking is required; when this occurs to follow the policy and proceduresrelating to lone working as provided by the practice.
Medicines management
Ensure safe storage, rotation and disposal ofvaccines and drugs is undertaken. Where appropriate, oversee the monitoring andstock control.
Unless authorised as an independent /supplemental prescriber, to only administer medications that have beenauthorised under either a PSD or PGD.
