Practice Nurse

About the Role:
We are seeking a highly skilled and motivated Practice Nurse to join our team at Wollaton Park Medical Centre. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the overall success of our medical centre.
Key Responsibilities:
- Provide and maintain a high standard of nursing care to patients, in conjunction with the nursing team.
- Focus on delivering evidence-based practice for patients with long-term conditions and providing preventative healthcare to the practice population.
- Provide nursing care to patients, including primary care treatment, preventative care, screening, and education.
- Support the practice in achieving local and national targets in respect of the PMS contract, QOF, and Enhanced Services.
- Communicate effectively with patients, carers, and other team members to provide quality nursing care.
Requirements:
- Registered Nurse with sufficient demonstrable experience post-qualification.
- GCSE grade A to C in English and Maths.
