Job summary

This is an excellent opportunity to take on a key clinical role in the delivery of the new Bromley All-Age Continuing Care (AACC) service. The role of Children's and Young People's Continuing Care (CC) and Transitions Nurse Assessor will be a good fit for an individual with strong clinical skills, excellent knowledge of children and young people's continuing care, children's services and Special Education Needs and Disabilities (SEND), and for someone with an ability to drive forward change and improvements in a critical and complex area of work.

The role is a good fit for someone who can balance the needs of extremely vulnerable children and young people, their parent/carers and advocates with that of the best-practice clinical delivery of a statutory service.

The Children and Young People's Continuing Care (CC) Lead Nurse Assessor will undertake assessments and reviews for children and young people in receipt of continuing care (CC) or coming into the service and to support with the transitions functions. Support the delivery of best possible clinical outcomes for eligible children and young people, planning for adulthood activities for eligible young people transitioning at age 18 and to ensure that CC is aligned with other CYP programmes of work such as Special Educational Needs and Disabilities (SEND).

Main duties of the job

The post holder will be responsible for:

acting as the lead nurse assessor for children and young people's continuing care (CC) with a responsibility to: lead and undertake effective CC assessments and reviews of children and young people. lead and undertake case management to children and young people eligible for continuing care (CC). liaise with clients, families, advocates and providers in relation to children and young people eligible for continuing care (CC). undertake innovative work to source particular and bespoke services (including assistive technology and equipment) for children and young people's CC to meet more challenging and non-typical needs. programmes and functions that support the delivery of the ICB Bromley statutory duties in relation to (i) the National Framework for Children and Young People's Continuing Care and (ii) the National Framework for NHS Continuing Healthcare and NHS-Funded Nursing Care. leading AACC clinical improvement programmes around quality, performance and service delivery for children and young people's continuing care (CC) in line with the postholder's responsibilities.

Education / Qualifications

Essential

Master's degree or equivalent professional qualification or management experience or knowledge acquired through experience or training Registered children and young people's nurse on the Nursing and Midwifery Council (NMC) register Experience working as a nurse assessor or equivalent for continuing care (CC)/continuing healthcare (CHC) Excellent knowledge of continuing care (CC) guidance, policies and decision making. Evidence of continued professional development Degree supplemented by diploma level specialist training, experience, short courses Registered children and young people's nurse on the Nursing and Midwifery Council (NMC) register

Experience

Essential

Experience or a strong understanding of the policies, procedures and practices employed by clinicians and social workers to assess and review eligible clients in receipt of continuing care, children's social care and SEND. Experience of providing excellent customer service in a public sector environment. Demonstrated experience of delivering best possible clinical outcomes for children and young people eligible for care services. Significant experience of effective work with children and young people patients/service users, families and carers to deliver the best possible clinical outcomes. Experience of managing and mitigating risks in a sensitive public sector type environment. Experience of providing expert advice and/or training in relevant areas.

Desirable

Experience in a clinical role in a relevant health or care setting. Experience of working with clinicians and/or within clinical settings to deliver excellent health outcomes for children and young people in care settings. Experience of working in the NHS and/or local authority at a relevant level. Experience of managing budgets and business planning processes Experience of chairing complex meetings across multi-disciplinary teams. Experience of successfully managing disputes between organisations or with clients and their families, including through recourse to legal avenues of redress where necessary.
