An exciting opportunity has arisen to join the Evelina Children’s Community Nursing Service as part of the Children’s continuing care team. The Continuing Care Team co-ordinates care into the homes of children and young people (CYP) who have complex life threatening/ life limiting conditions across the boroughs of Lambeth and Southwark. These CYP have a range of conditions and needs which can affect their ability to breath, move independently and communicate. We also support a growing number of children with behaviours that challenge.

We are looking for a passionate and enthusiastic individual with excellent transferable paediatric nursing skills, communication skills and a desire to advocate for children and young people with complex needs and their families within our community.

The Continuing Care team is part of the Children's Community Nursing Service, and we have a very close working relationship with all the teams within this service. We are primarily based at Sunshine House in Camberwell.

- To gather confidential, highly sensitive assessments and supporting evidence to be presented at the panel for consideration of funding under Children’s Continuing Care criteria which is documented in the National Framework for children and Young Peoples Continuing Care.
- To attend Panel when required deputise for the Team lead and to present required children to the Panel.

- To be able to work autonomously, working in the community setting and office environment.
- To oversee the day to day management of the caseload working closely with the Team Leads.
- Be responsible for regular contact with all families on the caseload.
- Be responsible for oversight of the “Equipment only” group of children, and to ensure that their equipment needs are being monitored.
- To lead on hospital discharge planning for children who have been offered a package of care, to liaise closely with wider MDT, care provider and family.
- To visit the child and family regularly post discharge and to be the point of contact for the family for the Community Nursing Service for the first three months post discharge (Monty Pathway)
- Independently manage a group of patients within the specialty whilst working as part of the multidisciplinary team, delivering specialised individualised and personalised direct patient care
- Demonstrate appropriate practice and specialist advice in the assessment, planning, implementation and evaluation of patient care. This includes history taking, physical examination and the requesting of agreed diagnostic procedures/clinical investigations.
- Responsible for the correct administration of prescribed medication including transfusion of blood and blood products
- Be responsible for assessing and recognising emergencies in the speciality. To interpret information and take appropriate action and to lead others to do the same.
- Act as an advocate for patients within the department, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.
- Ensure that patients’ receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines.
- Recognise and avoid situations that may be detrimental to the health and wellbeing of the individuals.
