Job summary

We are seeking a motivated and highly skilled Senior Physiotherapist to work withinourIntegrated Community Healthcare teams.

The CICH therapy team provides home based short term rehabilitation to a variety of patients, often with complex health conditions. We strive to provide first class, client-centred, goal focused therapy to patients in the community.

We also lead and provided intervention across all falls preventions pathways in Camden, and the role is expected to need high levels of expertise in falls prevention programmes

Working autonomously is essential, as our team is often faced with challenging situations which present themselves in the home environment. As a senior Physiotherapist, you will be working as part of an accomplished team to provide high quality, evidence-based interventions, whilst providing clinical support and having oversight of junior members of the team.

Following the NHS long term plan, CICH is proud to lead on putting neighbourhoods at the heart of integrated care. The service is set up to deliver health interventions within the framework of the neighbourhood model, working in partnership with other key primary care providers and the local voluntary care sector organisations

Main duties of the job

As a senior member of the Therapy Team, you will be expected to be a practitioner with a broad range of clinical competencies able to adapt to a variety of clinical circumstances.

You will be part of a community team where your proven ability to make high level clinical decisions will aid patient recovery and independence.

You will provide complex assessments and make high impact clinical decisions. You will be expected to work autonomously whilst drawing assistance from other colleagues in primary and secondary care.

You will use evidence based principles/current best practise and specialist skills to assess, plan, implement and evaluate clinical interventions and to ensure that all members of your team do so accordingly.

Your exceptional caseload management and communication skills will enable you to prioritise patient care and delegate to junior members of your team. You will work closely with other senior AHPs, community nurses, and other health or social care professionals to establish and maintain a whole systems approach for these patients

You will be expected to be a highly skilled Physiotherapist who is approachable, an excellent communicator, and able to adapt to the needs of both patients and service.

About us

Camden Integrated Community Health Service is a well established integrated team of therapy and nursing staff working closely together to achieve best patient outcomes in our community.

The role will be based across two Health Centres in Central London between two Integrated Neighbourhood Teams- West and North INT.

Regular CPD sessions and in-services are held, along with training opportunities identified from yearly appraisal.

As a Senior Physiotherapist you will have the opportunity to develop leadership skills through undertaking CNWL's 21st Century Leadership modules, as well as non-clinical duties to support the Therapy Lead.

As a leader you will have the opportunity to develop skills in managing people, and identifying and participating in service development through supervisory roles and QI projects.

There is much scope for development of both clinical and non-clinical duties within this role. Within Camden Integrated Community Health Service, we are proud of our high staff retention rate, and have played a significant part in the upskilling and of development of many clinicians within the team.

Essential

Bachelor Degree in Physiotherapy HCPC Registration Evidence of Continued Professional Development

Desirable

Post-Graduate or Master's Degree Member of CSP Leadership/Management courses

Previous Experience

Essential

NHS experience Previous experience to include the following; Frailty/Elderly rehab, Medicine and general surgery, Orthopaedics, Falls, Community working, Pain management, Neurology, Dementia, MSK, Respiratory, Experience of supervising students and junior staff

Desirable

Experience in falls prevention pathways Experience of working as an autonomous practitioner in a community setting

Other

Essential

Awareness of NHS polices in respect to primary care and reducing hospital admissions (including the NHS Long Term Plan)

Valid UK Driving License
