Main duties of the job

The role will require the Public Health Practitioner to:

Deliver effective strategic approaches to enable our residents to stay well, build resilience, reduce social isolation and loneliness across the life course, in line with the Health and Wellbeing Strategy priorities. Oversee the operational delivery and strategic direction of the in-house Social Prescribing service to ensure it achieves demonstrable outcomes for the population and reaches key target resident groups locally. Work collaboratively with key partners to ensure the public health programmes and Social Prescribing service are embedded into the wider health and social care system, as well as the existing community offer. Provide inspirational leadership to a small team of social prescribers.

Bracknell Forest Unitary Authority is appointing an experienced and dynamic Public Health Practitioner to join our team and who will have a range of complementary experience and skills. The person will be responsible for leading on the Adults and Social Prescribing portfolio. This will include the design, development, and evaluation of a range of key Public Health work streams, focusing on adults and communities. The role requires the Practitioner to lead the strategic direction of the social prescribing service, including management of staff and budget management.

Job responsibilities

To lead on the design, development and evaluation of a range of key Public Health work streams, in agreement with the Consultant in Public Health and Public Health Principal. The portfolio will be very closely aligned with Bracknell Forests current Health and Wellbeing Strategy, with a focus on the priorities 2 (Creating Opportunities for individual and community connections, enabling a sense of belonging and the awareness that someone cares) and 5 (Improve years lived with good health and happiness).

To oversee the operational delivery and strategic direction of the in-house Social Prescribing service to ensure it achieves demonstrable outcomes for the population and reaches key target resident groups locally.

To develop effective relationships with key stakeholders to drive the social prescribing offer to address social isolation and build capacity within the community, working closely with other services that support physical and mental health and wellbeing.

To work collaboratively with key partners to ensure the public health programmes and Social Prescribing service are embedded into the wider health and social care system, as well as the existing community offer.

Oversee the performance and monitoring of Social Prescribing and service and relevant public health programmes. This includes developing key performance indicators and reporting mechanisms.

Build and maintain strengths and capacities of voluntary, community and social enterprise groups to receive referrals and refer to the Social Prescribing and develop systems for evaluating effectiveness of referrals.

Contribute to the management of Public Health budgets aligned to Ring Fenced Public Health Budget and staff related to key Public Health work streams.

Identification of health needs, using appropriate analytic techniques and working with multiple qualitative and quantitative data sources, including intelligence from local communities

Supporting the development and delivery of Joint Strategic Needs Assessment and evidence- based strategies to meet these health needs

Evaluating the impact of programmes and projects using appropriate evaluation techniques.

To provide expert interpretational advice to local council colleagues (public health and non- public health) on the development and use of the public health outcomes framework.

To develop and manage the systems relationship between the public health function of the local council, Integrated Care System and Primary Care Network.

Maintain strategic links between the Social prescribing and Adult Social Care service and develop pathways to stimulate, manage demand and capacity, and ensure referrals are appropriate and timely.

Work closely with the council teams, Integrated Care Systems and Primary Care Networks to influence system and pathway development.

Use local intelligence on the needs of different communities to inform the public health prgorammes. This includes using appropriate analytic techniques and working with multiple qualitative and quantitative data sources, including intelligence from local communities.

Professional Responsibilities

Uphold the requirements of the UKPHR Registration, meeting the standards through public health practice.

Receive professional management and support from a Public Health Principal or relevant public health professional.

Undertake professional development activities, including in-house training, attendance and presentation at conferences and workshops and other agreed activities such as work towards registration as a public health practitioner

Work independently across the health and local council system to influence change and lead innovation.

Contributing to the training and supervision of other Public Health Practitioners and Trainees and Public Health Apprentices.

Managing public health projects, ensuring they are completed to time and budget, using project management methodologies.

Supporting and responding to bids for funds regarding agreed projects as necessary. This may include the drafting of proposals and demonstrating the analytical processes to be followed.

Managerial Responsibilities

Line manage a team of social prescribers to ensure the effective delivery of the service and professional capacity of the workforce to meet population needs.

Provide support to the Public Health Principal to ensure that effective systems for programme management are in place to support the delivery of programmes and ensure programme and project objectives are met.

Provide support to the Public Health Principal to develop and monitor appropriate performance indicators and outcome measures as agreed in the Directorate Business Plan.

The postholder will ensure that where they are collecting, calculating, recording, analysing or reporting data, it is accurate, reliable and consistent to ensure that decision-making processes are based on the highest quality data in accordance with the Council's Data Quality Statement. The postholder will also ensure that sensitive and personal data is handled in accordance with data protection legislation and Council GDPR policy.
