Job summary

Blackpool, Fylde & Wyre community NMC Registered Nursing Staff to undertake Full-Time Community Specialist Practitioner Programme in District Nursing (University of Central Lancashire), commencing September 2024.

An exciting opportunity has arisen within Families and Integrated Community Care Division to offer a place on the Community Specialist Practitioner (CSP) programme in District Nursing at the University of Central Lancashire (UCLAN) , commencing in September 2024. The CSP programme is undertaken on a full-time basis only and delivered over a minimum of 55 weeks, including fixed annual leave. In line with NMC changes, time in practice is now captured within Protected learning Time (PLT) and is 1065 hours minimum or 142 days based on a hour day practice. University attendance is 113 days, 59 taught days and 54 self-directed study. Please ensure you seek your manager's consent before applying - due to tight timescale you won't be able to complete your full notice period.

Main duties of the job

The community specialist practice programme will focus on four areas; V300, Research, Advanced Clinical Examination Skills and a Leadership and Management module. UCLAN will share their intended programme and timetable from September 2024 until September 2025 and this can be discussed on a one-one basis with anyone expressing a genuine interest in undertaking the course.

The course will be completed at level seven. Placement areas will be decided post interview.

On successful completion of your course you will be in the position to apply for any band six vacancy within community services. However, if there are no band six vacancies at the end of your programme you will be given a band five position within Fylde Coast Localities.

Some of the duties and responsibilities include:

Assess, plan, implement and evaluate the individual requirements of patients and carers referred to District Nursing Service.

Provide a high standard of evidence based nursing care to patients in the Community.

Support Team Leader when they are present. Designated to take charge of and lead the team in the absence of the Team Leader/Caseload holder, including day to day supervision of other team members, delegation of work and monitoring of nursing care. Provide support, guidance and clinical expertise as required.

Use a high level of communication skills with patients and their families, including those with English as a second language or learning disabilities, to communicate condition-related information.

Actively seek to promote health to patients and carers.

Administer drugs within NMC guidelines.

Attend team briefing and staff meetings, representing team in absence of Team Leader.

Keep nurse management aware of any factors which may affect treatment given to patients and produce reports in writing when requested. Report and assist in investigation of patient complaints.

Participate in achieving measurable standards of care and to participate in Clinical Audit.

Maintain accurate records in line with NMC guidelines.

Liaise with Primary Care Team, Professions Allied to Medicine and outside agencies in the delivery of care (including voluntary organisations/advocacy services) and refer as appropriate and attend Case Conferences deputising for Caseload Holder.

Responsible for safe use and authorisation of loans equipment use, following risk assessment, for example, pressure relief mattresses, hoists, etc. Ensure safe use of specialised equipment, Dopplers, digital cameras, ear syringes.

Offer telephone advice, using a problem solving approach, and follow up visits if necessary, to patients and relatives.

Record accurate and contemporary records (paper and electronic).

Follow policies and comment on policies and protocols to inform line manager of limitations, deficits or improvements. Has opportunity to sit on specific policy groups to represent grade or team.
