We are seeking to appoint a full-time Advanced Clinical Practitioner to support our evolving service.

Current hours of working are predominantly 9am till 5pm monday to friday with the potential for weekend work.

We have a multidisciplinary approach to providing care to our patients which involves Advanced Clinical Practitioners, District nurses, community nurses, OT's, PT'S, neighbourhood Practitioners, health care assistants and admin in the community.

As a qualified Advanced Clinical Practitioner, you will form part of the senior clinical team within Central Neighbourhood. Together with another ACP, matrons and a team of nursing, AHP's, social workers and support team members you will provide senior clinical support and leadership within the team

You will be involved in providing a responsive service to acutely unwell patients as well as some management of patients with multiple morbidity. You will also see patients with undifferentiated and undiagnosed problems, making an assessment of their health care needs, based on highly-developed nursing knowledge and skills, such as physical examination.

Further afield, Blackpool also benefits from less than three-hour connections via rail to Edinburgh in the north and London in the south.

Receive patients with undifferentiated and undiagnosed problems, making an assessment of their health care needs, based on highly developed nursing knowledge and skills, including skills not usually exercised by nurses, such as physical examination.

Screen patients for disease factors and early signs of illness.

Make differential diagnoses using decision-making and problem-solving skills.

Develop with the patient an on-going care plan for health and well-being, with an emphasis on health education and preventative measures,

Order necessary investigations and provide treatment and care both individually, as part of a team, and through referral to other agencies.

Have a supportive role in helping people to manage and live with illness.

Have the authority to admit or discharge patients from their caseload and refer patients to other health care providers as appropriate.

Work collaboratively with other health care professionals and disciplines. Provide a leadership and consultancy function as required.
