**Advanced Clinical Practice**: You will be responsible for providing specialist nursing advice and support to professionals working with the frail and elderly, whilst also supporting patients and their families to avoid unnecessary and very often distressing hospital admissions. You will work closely with our Physiotherapist who also has his qualification in Advanced Clinical Practice ensuring those who can benefit from the Living Well Services many rehabilitative and supportive sessions are referred in. An element of the role will be supporting the community team with your cohort of patients using Virtual Ward technology. You will be part of a rota system supporting the management plans of those patients who fall under the frail and elderly category.

You will be expected to maintain clinical credibility by acting as a clinical practitioner within your own area of expertise. You will contribute to, and influence the development, evaluation, and audit of this frailty project by working collaboratively with General Practitioners s, primary health care practitioners, statutory and voluntary agencies influencing the delivery of evidence-based care for the patients and families caring for the frail and elderly. You will lead on, and offer expertise and guidance to professional colleagues, families the general public and others in supporting the best outcome for the frail and elderly. You will be expected to take advantage of the clinical supervision and practice development available at Trinity and work alongside the Clinical Educator and Practice Facilitator to provide any necessary specialist training.

As an effective practitioner you will be able to demonstrate a number of core skills in order to support others as they learn. You will be able to give and receive timely and objective feedback whilst encouraging others to adopt a reflective approach to their daily work. You will be You will need to work collaboratively within the PCNs to influence practice relating to the support and treatment of the frail and elderly replicating good practice in those areas that are achieving the best outcomes for this group of patients. Good practice will need to be cascaded to those areas that are evidenced to have higher than average admission to hospital data whilst ensuring the staff are supported to feel confident with the support they can offer.

A key area of further development will be to achieve the Fylde Coast Strategic Aim that all patients with a palliative diagnosis will have an individual advanced plan of care developed in a caring and supportive way including the appropriate family members as required. This will involve developing Advance Care Planning Clinics to support professionals with the knowledge, skills, and confidence to be able to support those frail and elderly patients develop a plan of care that meets their needs including the limitations of treatment but also the goals, spiritual needs, and desires of the individual. 3.Leadership and Management: You will be expected to demonstrate excellent leadership skills, working as an autonomous practitioner supported by the Senior Clinical Team at Trinity to influence change of practice in different settings in relation to care of the frail and elderly. This will be made possible by providing the education, knowledge, and skills to enable high quality, safe care for the frail and elderly in the most appropriate environment.

You will assist in the development of practice, clinical protocols and policies for the clinical team Identifying, monitoring, and reviewing deficiencies in procedures and implementing remedial action. You will be required to facilitate the use of up-to-date information technology in order to collate accurate and timely information as and when required. You will have the skills to encourage and motivate others to be forward thinking in ideas that address the needs of the frail and elderly that will be innovative whilst managing and developing new ways of working, e.g., patient led advance care plan clinics, champions within each area etc. You will be expected to partake in clinical supervision for your own development.

You will Lead on setting standards and promoting best practice. 3. Research You will have the skills to have continuous involvement in research/evaluation and/or development work to identify the priority areas and from reliable data ensuring the most impact on improvement in the care of the frail and elderly. The role will require you to actively contribute to Practice/Service/Team audit/Research activity as required.

Human Resources Attend all appropriate training requirements both statutory and mandatory to comply with legislation. To comply with GDPR. Engage in regular supervision and PDRs in accordance with Hospice policy. Record Keeping To maintain records to meet Hospice guidelines, professional standards and standards set by statutory bodies such as the Care Quality Commission.
