There is an exciting new opportunity within the Neonatal Home Care Team. We are seeking dynamic, enthusiastic, motivated and experienced neonatal nurses to be part of our homecare team. These new posts will help to provide a comprehensive service by maintaining our existing 7 day working and contributing to the creation of a brand new home phototherapy service. Being a part of the Neonatal service at NUH means you will also work within the north hub region of the EMNODN. This will allow for opportunities to network across the north hub, access training and support and gain knowledge in other neonatal services within the network. This may also involve travel across the region.

Band 6 neonatal nurses based at NUH are required to promote safe, early discharge of babies from the neonatal units, transitional care and paediatrics. Families will be supported by providing ongoing care needs in the home environment, for example the provision of nasogastric tube feeding at home, management of oxygen in the home environment, providing ongoing care for surgical babies, and the jaundice management of the baby requiring phototherapy at home.

Candidates must have current experience of caring for neonates in a hospital or community environment, and must be QIS trained in speciality. These posts offer a fantastic opportunity to utilise and develop your knowledge and skills in caring for babies in the home setting.

These posts will support the existing outreach team in planning the care and transition from the neonatal unit, transitional care and paediatrics through a structured discharge process, promoting safe early discharge from hospital to home. You will liaise with other members of the multidisciplinary team and support parents in providing high quality care in the home environment.

Candidates will be expected to undertake a regular clinical shift on either unit, equivalent to 10% of weekly hours.

About us

Our current working hours are between 0800 - 1800, this service is currently going through consultation to extend the working hours to accommodate the new phototherapy service. The proposed hours of work will be between 0800 - 2000, which may be worked on either a 12 hour, or split shift basis. These opportunities within East Midlands Neonatal Operational Delivery Network (EMNODN) are to work within the North Hub of the region based at Nottingham University Hospitals NHS Trust (NUH) neonatal service and will include extensive travel across the region.

We take great pride in delivering high standards of care to our local neonatal population and are excited to create a network wide service to support vulnerable neonates on discharge from the neonatal service.

Job responsibilities

The post holder will ensure continuous provision of integrated neonatal transitional and community care to ensure safe, effective and high quality care is provided to babies and families. Planning and delivering seamless pathways of care for babies and families from the neonatal unit and transitional care facilities on both sites (City and Queens Medical centre campus) and working in conjunction with the multidisciplinary team, maternity and childrens services other community teams and services.

The post holder will plan and deliver inspirational family integrated care programmes with families of NICU babies and the neonatal in service, homecare and multidisciplinary team.

The post holder will be a role model providing regular direct clinical care using knowledge, skills and experience to support the implementation of family care on either campus to deliver service need and provide support for staff in the NICU setting.

Key job responsibilities

Clinical

Work at least 10% of time in unit based clinical practice, with the emphasis on delivery of a quality specialist service, developed through principles that are research and evidence based to enhance the quality of patient care.

Take responsibility for the assessment of care needs, the development of programmes of care and the implementation and evaluation of these to ensure the delivery of a high standard of care to babies and their parents/carers, liaising appropriately with the ward sister.

Be a visible point of contact for parents, visitors, relatives, and staff, acting as a resource for problems and able to clearly present the babies / parents point of view to others.

Act as the baby and families advocate ensuring that their best interests come first.

Improve and maintain standards of essential (basic) nursing care in the neonatal unit by working with the Ward Sister/Charge Nurse and using tools such as clinical benchmarking.

Ensure that patient care plans are regularly updated, and changes implemented. Ensure that nursing records and documentation are maintained and current.

Ensure high quality, evidence-based care is offered to babies and, in collaboration with the Ward Sister/Charge Nurse, identify the common issues which affect the quality of care, devising clear action plans to address any shortcomings ensuring agreed action is undertaken.

Provide specialist support to the clinical team in dealing with the complex needs of different client groups and when imparting or receiving sensitive, complex or contentious information.

Demonstrate a specialist level of knowledge and clinical practice within the clinical specialty, acting as a role model and resource of clinical expertise to all staff, babies and their carers.

Provide specialist advice and practice support to the multi professional team.

Ensure that all babies that are admitted into the service have access to an appropriate care pathway from admission leading to recovery and a well-planned discharge and have a smooth transition into the home.

Promote and demonstrate contemporary and proactive approaches with emphasis on family centred care, to neonatal intensive care nursing practice and in collaboration with other senior nurses to facilitate the advancement of this practice.

In support of neonatal transitional and homecare demonstrate expanded nursing practice in transitional care areas and outreach, according to NUH neonatal guidelines. This includes a program of neonatal nurse visiting at home babies and their families including providing a home phototherapy service

Facilitate a family centred and integrated care culture within the neonatal intensive care unit. Facilitate partnership with parents/carers in all aspects of caring for their baby in hospital and at home.

Training & Qualifications

Essential

RGN Level 1 (or equivalent) Current registration with NMC Post-registration qualification relevant to speciality Mentorship qualification and/or Mentorship and Teaching and assessing qualification NLS provider Competent in medicines management
