Laindon Medical Group has a fantastic opportunity for an experienced practice nurse to join our team on a full-time basis for hours per week Monday-Friday 8am - 6:30pm. Please note part time hours may be considered.
Due to expanding patient list, we are presently looking to expand our team. Whilst it would be desirable for candidates to have experience within practice nurse role, it is not essential as we are happy to tailor a training package for the right individual.
We are looking for outstanding practice nurses who want to embrace the changes in general practice but value the preservation of continuity. We can accommodate and help you develop skills, and we welcome part-time agile working. Come and talk to us, come and see what we are doing, come and join us.
Main duties of the job
Undertaking health assessments, administering medication and treatments, and performing diagnostic tests. Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition. Managing and treating long-term conditions, in line with national and local policies and the practice needs. Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments. Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations. Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad. Meeting the needs of patients presenting for opportunistic wound care. Hypertension review and management. Running asthma, COPD and diabetes clinics and reviews. New patient and health checks.

Our reach: We care for 18,500 adults and children across West Basildon and beyond. Our clinical team is made up of 4 GP partners, as well 5 GPs . Our GPs work alongside advanced nurse practitioners, practice nurses, health care assistants, social prescribers, care coordinators and our own in-house pharmacy team.

A relevant nursing degree. Current NMC registration with an active PIN. Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications; Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.
Experience

Evidence of clinical skills and experience in management of long-term conditions, vaccinations and immunisations, dressing and wound care, and contraception and women's health, family planning, cervical smears, the collection of pathological specimens including swabs etc, travel medicine and advice, diabetes management, respiratory disease management, cardiovascular disease monitoring and management. Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice. Knowledge, and experience of its application, in supporting long-term conditions. Insight and understanding of current issues in nursing and the wider NHS.
