We are looking for a Band 6 nurse to join one of our Community Nursing teams based in ABC PCN (Maidstone), to support the development of this service. The service operates seven days a week from 08:00 - 16:00, with occasional late shifts of 10:00 – 18:00 on a rotational basis. Weekend working is generally on a 1:4 ratio with days off during the week.
The team support patients registered with GP practices working in the Maidstone town and outlying villages within the ABC Primary Care Network.
Each team has two senior nurses to take responsibility for a defined geographical area, and provide clinical leadership to their Band 5, Band 4 and Band 3 staff. The team will work together to manage caseload patients delivering a high-quality service.
Staff will be required to drive their own car and are able to claim mileage costs and access the NHS car lease provision, following their probationary period.
Main duties of the job
Using the named nurse model, we want to allow nurses to spend more time with patients, promoting self-management where possible, and improving patient lives.

Successful applicants are required to have the qualification level specified in the person specification in order to carry out the job, however, we do welcome applicants from individuals wishing to further their knowledge and skills and to gain this recognised qualification, which we will support. If you are successful at interview but do not have the essential qualification for the role, we will employ you as trainee and will support you to gain this qualification. You will develop new skills and knowledge whilst working with an established team, gaining valuable work experience and continuing to get paid. Once you receive the qualification you will automatically move into the position advertised.

Person specification
Qualifications
Essential criteria
Registered health care professional (NMC registration). Health related degree and evidence of relevant post graduate development at Level 6. Evidence of Specialist qualifications relevant to area of practice (examples specialist practitioner district nursing, non-medical prescribing; mentorship, coaching, clinical expertise).
Desirable criteria
PTLLS/DTLLS, PGCE or Certificate in Education. Management or supervisory qualification or experience of study. Working towards Master level study.
Experience
Essential criteria
Minimum of 5 years’ experience in area of practice. Practical experience in managing health related projects/complex caseloads. Supporting others (mentor / educator / supervisor) experience.
Desirable criteria
Experience of leading and managing teams. Experience with the use of Microsoft Office and a variety of IT packages (E-Pay / E-roster/ AT-P/ RIO).
Knowledge
Essential criteria
Understanding of National Service Frameworks and NICE guidance to relevant to care setting and its application across health care services. Evidence of continued relevant professional development and implementation in practice within the last 12 months.
Skills and abilities
Essential criteria
Understanding of training needs in relation to post. Ability to manage time effectively, priorities and work under pressure to meet deadlines. Ability to work effectively with a variety of people at all organisational levels across different sectors including areas of social deprivation. Ability to communicate complex, sensitive information to individuals and groups. Strong influencing and negotiating skills.
