Integrating talking therapies within long term conditions care pathways in collaboration with our colleagues in Livewell and University Hospitals Plymouth NHS Trust. Staff development is key to us & there will be opportunities for further training such as the LTC & supervisor courses offered by University of Exeter.
Main duties of the job
Assess& support people with common mental health problems in the selfmanagement of their recovery. Complete patient centred assessments to identify areas where the person wishes to see change &make accurate assessments of risk to self & others.
Makedecisions on suitability of new referrals, adhering to the departments referralprotocols, refer unsuitable clients on to the relevant service or back tothe referrer as necessary; or determining stepping up the person for treatment at highintensity.
Provide a range of information & support for evidence based highvolume low intensity psychological treatments. This includes guided self-helpcomputerised CBT, information about pharmacological treatments.
Work closely with other members of the team to ensure the Stepped care model is consistent. This work maybe face to face, telephone or via other media.
Adhereto an agreed work plan relating to clinical time carried out per week in order to minimisewaiting times & ensure treatment delivery remains accessible &convenient.
Completeall requirements relating to data collection within the service. Keeping professional and coherentrecords of all clinical activity in line with Livewell clinical record keepingpolicy.
Attending and engaging with weekly caseload management supervision and fortnightly clinical skills supervision.
This role may not be eligible for sponsorship under the Skilled Worker route.

1. CLINICAL
Accept referrals via agreedprotocols within the service.
Assess and supports peoplewith a common mental health problem in the self management of their recovery.
