Lea Vale Medical Group have a fantastic opportunity for an enthusiastic and caring Practice Nurse to join our friendly team at our Oliver Street branch in Ampthill.

We are looking to add to our team with the addition of a highly motivated individual, who is forward-thinking and who strives to provide the highest levels of care to all of our patients.

We are a multidisciplinary practice that includes GPS, Pharmacists, Practice Nurses, Paramedics and Health Care Assistants.

Main duties of the job

The successful candidate will work as part of the primary healthcare team, ensuring the delivery of safe and effective general nursing care to the practice population.

Ideally the candidate should have some experience in primary care, however we are happy to provide training for the right candidate.

Job responsibilities

Principle Duties and Responsibilities

Clinical Care

Liaise and network closely with other colleagues within the Practice and with other Primary Care professionals, referring patients as necessary.

Ensure evidence based care is delivered at a high standard to patients providing expert clinical advice as appropriate.

To participate in the audit process, evaluating the service, implementing action plans and change of practice as necessary.

Implement, in accordance with practice protocols, treatment interventions for patients with long term conditions.

Plan, co-ordinate and carry out immunisations and vaccinations programmes in line with local or national policy.

Participate in and carry out other aspects of health screening, ensuring that targets are met.

Risk Management

Demonstrate clear understanding of risk including own professional accountability and responsibility, how to minimise risks in order to deliver safe and effective healthcare.

Monitor work areas and practices to ensure they are safe and free from hazards, and conform to health, safety and security legislation, policies, procedures and guidelines.

Ensure safe storage, rotation and disposal of vaccines and drugs.

Apply infection control measures according to guidelines.

Communication

Ensure effective communications with patients especially in providing and receiving highly complex, sensitive or contentious information where there may be significant barriers.

Experience in primary care Participation in research and audits and disseminating evidence-based practice Autonomous practitioner working independently and in conjunction with other health care professionals Experience of clinical governance management
