Greater Manchester Mental Health are currently looking foran experienced Social Worker to join their CMHT later life team at Etrop Court.If you are looking to develop your NHS experience and provide social care withthe support of working within a multidisciplinary team, then please do apply
Within this role, you will provide a person centred,statutory social work service for a caseload of patients in accordance withrelevant policies and procedures. You will be assessing and planning supportplans to meet individual needs as well as working with partners to safeguardservice users.

Conduct assessments to determine clients' needs and develop individual careplans
Provide counselling and support to individuals and families facing challenges
Liaise with external agencies and professionals to coordinate services forclients- Advocate for clients' rights and access to resources
Maintain accurate records of client interactions and progress
