Main duties of the job

As a Practice Nurse, your responsibilities will vary. You will

Qualifications required

Registered Nurse: Adult Nursing with current NMC registration Experience working autonomously as a clinical practitioner following best practice guidance and identifying risk to patients through sound use of clinical expertise Knowledge of clinical audit and its relationship to research Although not essential ideal applicants would have experience in a hospital, community or prison environment.

Job responsibilities

In order to meet theneeds of patients in this new prison Practice Plus Group has developed anexciting new integrated approach to the provision of care. Teams will worktogether at each stage of the patient journey, supporting them as they arriveinto prison (Early Days in Custody Team), delivering the care they need,promoting good health and supporting resettlement (Planned Care Team) andresponding to urgent and emergency needs (Unscheduled Care Team).

As a Practice Nurse you need expert clinical skills anddemonstrate judgement in assessing and managing wide ranging and complexpatient problems. You will provideclinical leadership for the Primary care teamand be a critical leader within the scheduled care Team. Thismulti-disciplinary team is made up of a variety of registered andnon-registered professionals including; nurses, doctors and mentalhealth/substance misuse practitioners. You will act as a rolemodel ensuring professional appearance and maintaining professional standardsat all times.

You will deliver face to face care and provide safe andeffective health assessments and treatment of patients within theestablishment.

You will be accountable for your own practice, includingthe assessment, diagnosis, treatment and discharge or referral of patients,independent of medical intervention.

To practice as a Practice Nurse within own scope ofclinical competence and agreed acceptable limits of practice. This is toinclude senior leadership in complex wounds, long term conditions and QoFmanagement.

You will work as part of the multi-disciplinary team inproviding quality, evidence based health care to meet the immediate needs ofthe patient.

You will also;

Provide o utstandingevidence based care, listening and acting on patient feedback. Ensuring thatcare is personalised and informed by what matters to them. You will also provide assessment,diagnostics, treatment and continued management of patients with acute and longterm medical conditions.

Deliver non-judgemental care byleading the team to deliver care that is safe, high quality, patient centredwhilst meeting CQC requirements following our policies, procedures and alwaysconsidering safeguarding.

Inspire excellence in yourcolleagues by pro-actively leading andmotivating the team recognising that our workforce is our most valuableasset. You will provide professionalleadership to the nursing team to ensure we provide a service that people cantrust, feel safe within and feel proud of.

Promote best practice, clinicalsupervision, evidence based care and continuous quality improvement embedding aculture of reflection and shared learning.
