Job summary

The Norfolk and Norwich University Hospital is a forward thinking teaching hospital offering acute care to a population of approximately one million. Electronic prescribing and medicines administration has been implemented across all inpatient wards, the A&E and outpatients. We are at the forefront of healthcare delivery with a strong focus on Medicines Management and innovative pharmaceutical practice.

In recognition of the pivotal role a pharmacist can have in relation to reducing risks associated with anticoagulation and thromboprophylaxis an exciting NEW POST for an advanced pharmacist has been established to work alongside the Thrombosis and Thromboprophylaxis Committee at the NNUHFT.

This post would suit an experienced pharmacist, who will strive to improve quality, safety and governance relating to all aspects of anticoagulant / thromboprophylaxis use across the Trust and wider ICB.

Hours of work will be Monday to Friday with a requirement to work 1 weekend in 10 with corresponding time off during the week. There is also a requirement to work at least one public holiday per annum.

Main duties of the job

Previous experience of working in this setting is desirable but not essential. A key early priority of the role will be to lead a LMWH heparin switch across the Trust.

The successful post holder will be involved in writing and reviewing Trust policies and guidelines in relation to thrombosis and thromboprophylaxis as well as training of all staff groups in relation to use of these agents. Owing to our close links with the UEA, opportunities are available to get more involved in research. Should you be successful in securing this role you will join a friendly pharmacy team committed to providing high quality pharmaceutical care.

Although your role will be primarily linked to leading medicines optimisation of thrombosis and thromboprophylaxis, you will input into the clinical pharmacy service of a named speciality, attending ward rounds and supporting junior pharmacists, pharmacy technicians and trainees.

Youwill be self-motivated, able to work independently and be capable of working in a fast-paced environment. Positive demonstration of the Trusts PRIDE values is a pre-requisite. We would support professional development of advanced practice roles, leadership capabilities and completion of an independent prescribing qualification if you do not already have one.

Job responsibilities

Specialised Clinical Pharmacy Activity

1. To act as a specialist pharmacist providing an advanced level clinical pharmacy service to an Anticoagulant and Thrombosis Service, identifying and helping to manage the more complex/high risk anticoagulation patient population on the wards, optimising patient care with respect to medicines for patients in liaison with health care professionals, including medical and nursing staff.

2. To communicate highly complex drug information to prescribers, clinicians, patients and/or relatives (who may sometimes be distressed and have language difficulties or other barriers), nursing and pharmacy staff.

3. To develop a highly specialist knowledge of thrombosis and thrombo-prophylaxis and treatment of co-morbidities to promote and support evidence-based medicine within thrombosis and thromboprophylaxis and across the Trust in order to fulfil the role as a pharmaceutical lead

4. When suitably qualified, to prescribe and deprescribe medicines for agreed patients within their capacity as an Independent Prescriber in line with current legislation, local policies and within their own level of experience and competence. Provide an active prescribing role within Thrombosis & Thromboprophylaxis service clinics as required by the service.

5. Actively participate in multidisciplinary ward rounds, Thrombosis & Thromboprophylaxis specialist team MDT meetings and/or clinics for Thrombosis & Thromboprophylaxis patients, providing advanced pharmaceutical care to inpatients and outpatients.

6. Use appropriate patient record system(s) to view and record clinical activity.

7. To support any future development of Electronic Patient Records.
