Pioneer WHLC is excited to announce two vacancies for Tissue Viability Nurse Consultants as we launch our new flagship centre of excellence. To support this exciting expansion, we are actively recruiting skilled professionals to join our team.

Speciality: Wound healing and lymphoedema.

Main duties of the job

The NurseConsultant is responsible for the leadership and effective delivery of patientcare for Tissue Viability/lymphoedema. Recognised as a clinical leader and advancedpractitioner, they act as a key advisor for advancing practice for theirclinical team.

Tolead/manage the clinical service in an efficient and effective manner whilstensuring that the culture, standards and outcomes are developed, improved andmaintained.

To promoteclinical excellence through the provision of consistent specialised care,skilled communication and continuous professional development of staff.

Toundertake responsibilities designated within the role related to the CQC.

To workcollaboratively to implement audit processes and systems within Pioneer for thepurpose of promoting clinically effective and continuously improving care.

Job responsibilities

Key responsibilities and accountabilities:

Clinical

Plan, co-ordinate,deliver and evaluate patient care and ensure safe discharge of patients inaccordance with Pioneer Discharge Policy

To provideprofessional leadership and demonstration of competent advanced practice intissue viability acting as the lead clinician who supervises staff. Assessing,planning, implementing, overseeing and evaluating programmes of care andadvanced skills.

Respect peoplesdignity, wishes and beliefs; involve them in shared decision-making and obtaintheir consent before undertaking procedures

Promote a patientfocused multidisciplinary approach to care in collaboration with colleagues, carers,health care professionals and other agencies

To work according to the NMC Code of Professional Conduct and relevant professionalguidelines as the Lead clinician for a defined group of patients

Communication Core Dimension

Use communicationskills in order to convey complex and sensitive information effectively topatients, carers and other staff, overcoming any barriers to understanding andproviding support during distressing or emotional events.

Maintainprofessional boundaries and working relationships with patients and colleagues.

To act aspatient/staff advocate through the application of ethical, legal andprofessional knowledge and skills.

Educate patients and their carer(s) asrequired where a need has been identified.

Be an effective communicator both internally within Pioneer andexternally with all relevant parties recognising that the role of NurseConsultant is often a prominent figure head of the organisation.

Personaland People Development

Develop staff within a defined group viacoaching and mentoring. Undertake talentmanagement of clinicians/Healthcare support workers through recruitment,engagement, staff development and performance management.

Ensure that all staff attendorganisation/local orientation programmes, mandatory training sessions andannual updates.

Be responsible for developing and sustainingown knowledge, clinical skills, reflection and appropriate learning frompractice. To maintain and develop appropriate competence, performance andprofessional awareness in accordance with CPD and personal professional profilerequirements, this includes being appraised at least annually and contributingto own personal development plan (PDP).

Identify the learning needs of key staffwithin the wider workforce, leading on professional development of the advancedpractitioner workforce. Undertake the training, development and competencyassessment of all clinical staff and complete assessors training as required.

Health,Safety and Security

Maintain an open and transparent safetyculture by embracing and embedding Freedom to Speak up Principles.

Take responsibility for promoting a safeenvironment and safe patient care by identifying areas of risk and followingthe Incident, Serious Incidents and Near Misses reporting policy and procedure.This includes identifying potential risks for all staff, patients and visitors andadhering to relevant incident reporting procedures.

To ensure compliance with Pioneer policies for Clinical,Corporate, Health and Safety, Information Governance and Human Resources, personaland patient safety, prevention of healthcare-associated infection (HCAI); thisincludes hand hygiene, use of personal protective equipment and safe disposalof sharps and clinical waste.

To ensure that all Pioneer Risk Management systems and processesare implemented to reduce risk to its lowest practicable level; this includesraising risk awareness, seeking risk and safety advice and escalating risksthat cannot be managed locally to the Senior Management Team and Pioneer Board.

ServiceImprovement

Participate in the development, implementationand monitoring of nursing practice, standards, guidelines, protocols, policiesand care pathways.

Instrumental in the setting and monitoring ofmeasurable standards of care and be accountable for maintaining standards.

Promote and deliver nursing/clinical practiceand care delivery in line with relevant current validated research evidence,best practice when available and with reference to Pioneer policies andguidelines.

Contribute and manage research and developmentprogrammes.

Develop and maintain resource management skills which includes takingresponsibility for ensuring that Pioneer resources are used efficiently andwith minimum wastage.

Quality

Promote a happy and supportive environment forstaff.

Maintain own and others awareness of thenational, professional and local quality issues relevant to the delivery ofnursing services.

To monitor and audit standards of all carewithin all clinical settings as agreed with the Clinical Management Team

Providing monthly reports to the SeniorManagement Team

Ensure standards of clinical practice, recordkeeping and reporting are of a required standard and any issues identified areescalated expediently.

Uphold quality initiatives that improvecustomer care and enhance the interface between staff, patients and visitors.

Participate in recruitment and selection ofstaff after undergoing appropriate training

Undertake appraisal of designated staff,addressing personal development plans (PDP) and monitoring/managing performance

Contribute to directorate/divisionalmanagement processes by being a member of the Clinical Leadership Team

Safeguarding: Provision of care to meet healthand wellbeing needs

All staff must be familiar with, and adhereto, Pioneer child and adult protection procedures and guidelines, inconjunction with the Local Safeguarding childrens board (LSCB) and Adultssafeguarding board (ASB) policies and procedures.

Ensure that all clinical staff are familiarwith their role and responsibility around safeguarding children and adults andto ensure that they have completed training at a level commensurate to theirrole.

Records and reports the interventionsconsistent with legislation and relevant policies and procedures.
