Job summary

Hospital at Home Frailty have a exciting bank opportunity for a practitioner with advanced skills to join the team.

Our service covers the urban and rural areas of Somerset, predominantly based in one area however cross cover, especially at weekends, will be required.

Hospital at Home operate between 8am and 8pm we offer a variety of shift within our rota.

We offer flexible working solutions where possible to ensure a work-life balance. Weekend cover is required as part of the role.

If you're a qualified practitioner with advanced skill looking for a rewarding challenge, join our friendly, and highly skilled team of trainee ACP, Medics and support staff. We can offer you training and supervision to support and guide you in your new role.

Advanced skills such as non-medical prescribing are essential for this role.

You'll need to be a car driver with your own vehicle to visit patients. We offer a generous mileage allowance to cover the use of your car for NHS business, which includes both mileage and depreciation.

Main duties of the job

Hospital at Home enabling people to receive acute care and treatments in home surroundings with support from a team of health and care professionals.

Attend MDT and huddles to discuss patient caseload and plan intervention.

Carry out autonomous assessment and treatment plans for patient within the caseload , manage expectation of patients.

work within scope of practice and seek opportunity for learning from other within team.

Open and honest communication with patients, family and stakeholders.

Prioritise own time and support team to ensure caseload is managed.

Build relationship with neighbourhood teams to support patient to remain within own home.

Work with Internal and external teams to promote Hospital at home.

Participle in service development , identify own needs and support training to team and other teams in directorate.

Ensure as a ACP your meeting the 4 pillars of the role.

Job responsibilities

Hospital at Home- enabling people to receive acute care and treatments in home surroundings with support from a team of health and care professionals.

To be responsible for the rapid assessment, diagnostics, and treatment of individuals within the Hospital at Home service using an advanced level of professional accountability, autonomy, and judgement, underpinned by Masters level theory and experience. This includes highly complex decision making across a broad range of differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence based judgements and diagnoses,

To plan and manage complete episodes of care. This will be working in collaboration with senior medical colleagues and others when required. Delegating and referring as appropriate, to ensure timely, effective management to optimise health outcomes, in line with the evidence base and nationally recognised best practice.

To play a key operational role and maintain a pro-active approach to managing, updating and leading clinical pathways.

To promote and implement service and policy development and redesign, impacting beyond own area and other disciplines, informed by current best practice and evidence base, in accordance with values-based care

Person Specification

Qualifications

Essential

Professional Registration with either NMC, HCPC Evidence of MSc level education. Such as: MSc Advanced Practice. Equivalent other MSc mapped to the four pillars of advanced practice. Demonstrated competence at an advanced practice level through Royal College or HEE Credentialing Relevant role specific qualifications ( Advanced Resuscitation ) Leadership management course or validated portfolio evidence (if not part of MSc level qualification) Evidence of Continuing Professional Development Completed ACP training within relevant clinical field Non-Medical Prescribing if appropriate to registered profession role.

Experience

Essential

Significant experience of working as a registered practitioner in a provider organisation ( acute sector, community provider, third sector, social care or primary care) and within a relevant specialty (suggested 4-5 years minimum) Evidence of recent teaching, mentorship and supervision of staff Evidence of recent audit and/or research experience Experience of multi-professional working Ability to interpret and evaluate national and international guidelines and apply to local protocols and practice as appropriate Able to use high-level problem-solving and analytical skills and manage complexity Effective time-management and prioritisation skills Evidence of theoretical and practical skills allowing autonomous patient management ( patient assessment, diagnosis and management) related to clinical scope of practice Understand, encourage and support clinical supervision and mentorship both within and across professional roles Effective leadership skills relevant to role ( demonstrated ability to provide clinical professional leadership) Effective written and verbal communication skills Educational skills such as identification of learning needs, delivery and evaluation, providing work based learning

Evidence of involvement in change management and/or clinical service improvement

Works under own initiative with strong organisational skills and ability to achieve to deadlines Appropriate level of literacy (, for radiology, pathology, microbiology ordering as per Trust protocols, utilising patient systems) Willingness to use technology to improve standards of care and support to our patients Access to a vehicle for travelling within and outside the trust boundaries
