Position Overview

We are seeking an Advanced Musculoskeletal Clinician with a strong focus on spinal disorders to become a vital member of our healthcare team. The ideal candidate will demonstrate the ability to operate independently as well as collaboratively, delivering exceptional evaluations for patients referred by general practitioners and physiotherapists when initial management strategies have proven insufficient.

Exceptional communication abilities are crucial, alongside a comprehensive understanding of spinal structure, related conditions, and suitable treatment pathways.

This position is offered on a whole-time equivalent basis, but we are open to discussing potential job-sharing arrangements or how this role could integrate with other opportunities within the Musculoskeletal (MSK) pathway.

Key Responsibilities

As an Advanced Clinician in the realm of musculoskeletal health, the successful candidate will provide specialized assessments, diagnoses, treatments, prognoses, and guidance to a varied and intricate patient demographic within their area of expertise.

The responsibilities include:

  • Conducting clinics to evaluate new referrals from general practitioners, first contact practitioners, specialists, physiotherapists, and podiatrists.
  • Possessing the capability to initiate advanced diagnostic procedures such as x-rays, blood tests, and MRI scans.
  • Exercising autonomy in decision-making, often advising medical professionals on suitable treatment options.
  • Ensuring comprehensive patient evaluations, delivering diagnoses, and discussing management alternatives, including surgical considerations for complex cases.
  • Serving as a knowledgeable resource for clinical colleagues, including physiotherapists, podiatrists, and general practitioners regarding musculoskeletal and orthopedic matters.
  • Filtering and managing referrals within the integrated MSK service, overseeing the care of patients who would typically be referred to hospital specialists.

Job Responsibilities

In the role of Advanced Clinician within the Orthopedic Assessment Service, the post holder will:

  • Manage patients autonomously, holding full responsibility and legal accountability for clinical decisions and actions.
  • Possess extensive knowledge of musculoskeletal conditions, with a specialization in one primary area, such as upper limb, lower limb, foot and ankle, or spinal health.
  • Conduct initial medical assessments and triage patients referred to the Orthopedic Assessment Service by general practitioners and hospital specialists, addressing a range of complex musculoskeletal issues.
  • Evaluate intricate musculoskeletal cases, considering:
    • The clinical characteristics of the patients.
    • The often lengthy and misleading history of the patient's condition.
    • The potential for severe anxiety or aggressive behavior stemming from the patient's pain state.
  • Adopt a holistic, patient-centered approach that incorporates psychological, social, and medical factors, referring to other specialists as necessary.
  • Encourage straightforward management strategies for long-term conditions for use by general practitioners and other healthcare professionals.
  • Exhibit highly developed physical skills requiring precision, such as administering intra-articular injections and performing manual therapeutic techniques.
  • Request specialized investigations, including x-rays, ultrasound, and MRI scans, as appropriate.
  • Clearly communicate the results of investigations and clinical diagnoses, including prognosis and potential surgical needs, when necessary.
  • Understand and utilize the referral process to other health professionals or agencies, requiring specialized knowledge across various medical conditions.
  • Refer patients to other specialists, such as neurologists, rheumatologists, vascular surgeons, oncologists, or pain management experts, as needed.
  • Implement suitable treatment programs, including listing patients for surgery, both major and minor, with only the most complex cases requiring discussion with consultant colleagues.
  • Provide specialized guidance to assist patients in managing and accepting chronic pain or reduced function, emphasizing an evidence-based biopsychosocial model of care.
  • Offer expert advice on physiotherapy and podiatry management.
  • Refer patients for physiotherapy, occupational therapy, or podiatry as appropriate.
  • Follow up with patients in clinic after investigations and/or surgical procedures.
  • Conduct all interventions within the scope of practice and in accordance with local and nationally recognized guidelines and standards.

Qualifications

Essential

  • Degree or diploma-level education leading to State Registration by the Health and Care Professions Council (UK) as a Physiotherapist or Podiatrist, or other relevant professional qualification and registration.
  • Competency assessment in musculoskeletal skills, including imaging training and diagnostic interpretation.
  • Significant post-graduate education relevant to musculoskeletal care, equivalent to at least a Master's degree.

Experience

Essential

  • Extensive experience in managing a variety of musculoskeletal conditions, with a focus on a specific specialty.
  • Experience in teaching musculoskeletal medicine as part of a clinical role.

Desirable

  • Management experience within the healthcare sector.
