Job summary

We are looking for an enthusiastic, motivated and competent clinician to become part of our friendly team at the Urgent Treatment Centre (UTC) in the New Forest, Hampshire. This is an exciting opportunity for an experienced clinician to undertake a new role for the department, with a focus on the initial assessment and care of patients that attend the UTC. The role involves working alongside experienced clinical practitioner prescribers to provide a high standard of clinical triage for patients attending the UTC, particularly during busy periods.

Main duties of the job

The successful applicant should already be a registered nurse or paramedic with good experience in caring for acutely unwell patients, as well as a good understanding of minor illness and injury presentations. The applicant will need a high degree of enthusiasm and drive to develop as a clinician and work with the team to gain the necessary knowledge and experience to provide safe and effective triage.

This is a unique opportunity for a clinician to gain a good experience and understanding of the urgent care environment, with a potential opportunity to apply in the future for the 'Development Clinical Practitioner' role within the UTC.

Job description

Job responsibilities

Work autonomously and be accountable for his/her own professional actions. Provide initial assessment for undifferentiated patients with a range of acute, non-acute and chronic medical conditions, as well as varying degree of injuries. Work within the scope of the IAC clinician as directed by the UTC clinical manager. Take a patient history, make appropriate physical examinations, formulate differential diagnoses and formulate/manage treatment plans when working within the UTC. Refer and facilitate patient pathways through appropriate referrals to primary, community, secondary, and tertiary care for patients when necessary, such as 2-week wait, urgent and routine referrals to specialities. Issue certain medication under PHL patient group directives (PGD) as allocated to the IAC role by the clinical manager. Recognise the need and initiate emergency treatment to save life or to safeguard the interests of the patient and refer to emergency services when appropriate. Recognise any social needs, safeguarding concerns and act appropriately to safeguard the patient. To ensure that patients are safely discharged from our care with appropriate instructions for after care and follow up. Ensure an excellent and consistent standard of patient documentation in keeping with the UTC audit standards. Adherence to KPIs without jeopardising the safety of the patient. Identify opportunities to proactively encourage health promotion and health education to all patients. To be professionally and legally responsible and accountable for all aspects of clinical decision making within your clinical competence. To take an active role within the cross-disciplinary, multi-skilled team to ensure service quality in line with corporate goals and strategies and provide mentorship and support to junior staff when required. Take a person-centred approach to service delivery, based on clear understanding and experience of the modes for service delivery. To assist in the development of new models for service delivery which have a positive impact on the health economy. Maintain and develop clinical competence and knowledge of care delivery within the health care setting, taking responsibility for your educational and developmental requirements and continued professional development. To work within the professional Code of Conduct and other directives as defined by the NMC/HCPC. To be responsible and accountable for legible, contemporaneous clinical records and engage with any feedback from the regular UTC clinical documentation audits. To undertake appropriate personal and professional development to meet the requirements of NMC/HCPC Registration. To participate in regular performance reviews with your professional line manager. Qualifications

Essential

Registered with the NMC/HCPC with no disciplinary sanctions, pending or current investigations. Degree level qualification in paramedicine or nursing. Level 3 safeguarding of adults and children. Must be able to demonstrate and meet competencies of all mandatory training as required within the policies of the organisation. Must be authorised to work in the EEC. Must be able to clear an enhanced DBS check.

Desirable

Teaching and mentorship qualification. ILS/PILS/ALS qualification. Completed or currently undertaking MSc ACP modules.

Experience

Essential

At least 2 years post registration experience. At least 2 years full-time working in an emergency department or pre-hospital setting. Good knowledge and experience in the assessment and management of minor injury and illness presentations. Experience of working in a multi-disciplinary team. Good organisational skills in the ability to manage and prioritise own workload. Excellent communication and interpersonal skills. Clear understanding of professional responsibilities and accountability.

Desirable

Accident and emergency or urgent treatment centre experience. Autonomous triage experience. Good knowledge of QOF and locally commissioned services.
