As a result of the previous postholders partial retirement, we are wishing to recruit a RGN to work as a part time Lead Nurse in our small and very friendly Continuing Healthcare Team.

This is a role for a Registered General Nurse, to support the team, clinically and operationally, on a part time basis; in line with the requirements of The National Framework for NHS Continuing Healthcare.

The role is varied and interesting, working collaboratively with health and social care colleagues in Wolverhampton to support some of the most complex and vulnerable people in our community.

It offers the successful candidate the opportunity to develop their knowledge of commissioning, whilst maintaining their clinical knowledge and managing a clinical team.

We offer a comprehensive induction, with ongoing development and support from a clinically lead team.

Main duties of the job

The post holder will need to be motivated and focused on providing compassionate and supportive service delivery for the NHS Continuing Healthcare process.

They will also need to display inspirational leadership skills and be a role model for team, who will require excellent time management skills and the ability to work autonomously.

They will jointly line manage the clinical team which will include regular supervision and PDRs.

The post holder will need excellent communication skills and the ability to adapt such skills to communicate with a wide variety of people, including patients and their families.

They will be required to provide high-quality professional advice, guidance, and expertise to members of the Individual Care Team, ICB and wider health and social care economy.

To ensure that assessments are holistic in nature and utilise The National Framework for NHS Continuing Healthcare. Verification of completed assessments to ensure quality and consistent approach.

To have an expert level of knowledge regards The National Framework and its application in practice. To be able to impart this knowledge to a variety of audiences.

To provide complex and sensitive information to patients, relatives/representatives, and work in potentially hostile environments, relaying difficult decisions regarding care provision, managing patient and family expectations.

To support the Head of Continuing Healthcare in developing and improving service delivery.

Experience of multi-professional collaboration at a senior level working in partnership with other organisations/agencies. Case management experience of community patients. Significant experience of successfully operating in a politically sensitive environment.
