An opportunity has arisen for a permanent, full-time, Band 6 Senior Community Psychiatric Nurses to join the Complex Care Team; a mental health community service in Wolverhampton.

The Complex care service is based at Penn Hospital in Wolverhampton. We are a community based service and offer treatment and support for individuals with a severe and enduring mental health difficulties. These

are typically care clusters 8 and 11-17 . These individuals are likely experiencing mental health difficulties such as Psychosis, Bipolar, depression & anxiety related disorders and personality disorder

Main duties of the job

We seek highly a motivated individual who is looking for a new challenge and who has the skills to make a difference with our service users.

Applicants must be RMN qualified and have substantial post registration experience in working with working age adults who experience acute severe and enduring mental health difficulties, within the area of specialty.

The successful candidate will be able to deliver high quality and safe patient care in line with CPA and discharge planning as they will be instrumental in working with and developing processes that support our service progression. The Candidate will also have an interest and knowledge of Personality Disorder.

You will be committed to a Recovery focused way of working, inherent within the team culture, empowering and instilling hope to our service users.

Professional:

To adopt and co-ordinate a multi-disciplinary agency approach to care, ensuring that all aspects of the patients needs are met.

To set professional standards of care and to monitor, measure and audit the effectiveness of care given.

To ensure an effective communication mechanism is in place, particularly with regard to patients care and progress, to include patients and relatives.

To deploy, supervise and support other members of the nursing team in the provision of a professional nursing service.

The utilisation of available resources and services within the community setting, identifying any inadequacies and taking appropriate action to enhance the quality of care available within their client group.
