Job summary

An exciting opportunity exists for a highly driven forward thinking experienced Critical Care Outreach nurse to join our innovative team.

The Critical Care Outreach Team is based within the Heart and Lung Centre at New Cross Hospital, and provides a 24/7 service across the Trust, covering approximately 840 beds.

We are looking for a motivated, dedicated experienced Critical Care Outreach Nurse who is committed to delivering high standards of safe, effective and dignified care.

The successful candidate will have a high level of knowledge and act as a specialist resource in the management of the deteriorating patient. They will be able to work independently, develop a good rapport with key leaders, clinicians, and have some influencing skills to improve patient outcomes and experiences.

As part of a specialist nursing team you will be required to work flexibly, to provide 24 hour care, seven days a week service.

The post holder will be responsible for maintaining, improving, and developing high standards of care, through education, and training, to improve patients outcomes.

As a band 7 critical care outreach practitioner you will have a strong work ethic, and the ability to work with members of the multi disciplinary team.

Excellent communication skills are essential to facilitate the effective multi-disciplinary approach in the early recognition, escalation and management of the deteriorating patient Trust wide.

Main duties of the job

To demonstrate expert and extensive knowledge in areas such as diagnostics and interpretation of results in order to develop a management plan of care.

Utilising enhance skills in areas such as consultation and clinical decision making in a variety of settings.

To work autonomously to ensure safe and effective clinical practice.

Provide specialist support, advice and education to the multidisciplinary team to ensure optimal management of the deteriorating acutely ill patient.

Job description

Job responsibilities

To be professionally accountable for the maintenance of standards of professional practice and behaviour as set by the Nursing & Midwifery Council, national legislation and local standards.

To take personal responsibility for actions and omissions, and fully recognise personal accountability.

To practice within an ethical framework based upon dignity and respect for the well-being and safety of patients and clients.

To demonstrate expert and extensive knowledge in areas such as diagnostics, therapeutics, the biological social and epidemiological sciences and enhanced skills in areas such as consultation and clinical decision making in a variety of settings.

Undertake interventional procedures/high specialist diagnostics with appropriate consent.

Act as a change agent, developing clinically effective practice through the effective utilisation and integration of evidence-based practice, setting implementing and monitoring evidence-based standards of care, policies, procedures and protocols.

Ensure a culture of continuous quality improvement through the use of audit, patient feedback and reflection on practice by self and other member of the team.

Promote a clean and safe environment for staff, patients and visitors by ensuring compliance with legislation, policies and protocols including health and safety, healthcare associated infection prevention, clinical governance, including risk management and critical incident reporting and root cause analysis.

Ensure a high standard of compassionate clinical care and record keeping in accordance with Nursing and Midwifery Council, national legislation and local standards.

Lead effective communication with the multi-professional team regarding patient care.

Assess the risks involved in the care of patients and ensure utilisation of practices and protocols to minimise those risks, including safe use of medical devices.

To monitor the standards of care and take appropriate actions when standards fall below expected levels.

Reports incidents and near misses promptly and appropriately and take effective action to minimise future risk.

Identify and implement systems to promote contribution of advanced level nursing and demonstrate the impact of advanced level nursing to the healthcare team and the wider health and social care sector.

Continually evaluate and audit the practice of self and others at individual and systems levels, selecting and applying valid and reliable approaches and methods which are appropriate to needs and context, and acting on the findings.

Continually assess and monitor risk in own and others practice and challenge others about wider risk factors.

Critically appraise and synthesise the outcomes of relevant research, evaluations and audits and apply the information when seeking to improve practice.

Registered Nurse with current NMC registration Degree Level qualification Teaching and assessing qualification or equivalent Possession of ENB 100 post 18 months qualification

Critical care experience including experience as a band 6 critical care outreach practitioner Evidence of professional updating within last 12 months
