We are seeking to recruit :

Someone with a passion and interest in learning disability and autism Someone who wants to make a difference within patient care and experience

Is this you ? do you want to learn and develop and help improve patient care within the Trust . Working as part of the Safeguarding Team in the Vulnerablities Team.

Opportunity for a band 5 or band 6 to step up into a band 6 learning disability liaison role for a period of 12 months

This could be AHP or nursing Up to 30 hours seconded/fixed term position ( eg:-working Tues- Fri) Opportunity to work in an innovative and supportive environment Opportunity to increase knowledge base in learning disability and autism Experience of working as a band 6 Learning opportunities - development both personal and professionally.

Main duties of the job

To work within the Learning Disability and Autism team and collaboratively across the Multi-disciplinary Team (MDT) to increase access and reduce health inequalities for people with learning disabilities and autistic people who use services at the Royal Orthopaedic Hospital (ROH). This post is for a fixed term to cover service provision due to maternity leave.

You will be working with a substantive Band 6 Learning Disability Nurse and supervised and suported day to day by the Trusts Senior Named Nurse for Safeguarding and the Trusts Safeguarding Lead Nurse.

The role will include:

Monitoring team emails Sharing information internally and externally Supporting the team to develop attendance and admission plans Supporting management of the service Supporting with monitoring and actioning of notifications Supporting staff to identify and implement reasonable adjustments Engaging with patients and families Supporting and engaging staff Creating road show displays

Job description

Job responsibilities

To work with patients who are diagnosed with learning disabilities, profound and multiple learning disabilities or autism, their parents, and carers to develop plans of care for both outpatient services and elective admissions ensuring the provision of safe, high-quality services.

To work as part of the Learning Disability and Autism Team, and wider Safeguarding and Vulnerabilities Team to provide specialist learning disability and autism support to patients to reduce health inequalities.

Work as part of a multi-disciplinary team (MDT) under the supervision of the Senior Named Nurse for Safeguarding to assist in the planning, implementation, and evaluation of comprehensive plans of care pre-operatively, during admission and post-operatively ensuring a safe and effective discharge using a patient-centred approach.

To actively promote the use of the patient held hospital passport and staff use of the internal learning disability and autism notification system.

To capture patient voice through regular engagement.

Create links with local hospital and community learning disability teams.

To work as a team to raise the profile of the health care needs of people with learning disability across the Trust.

Person Specification

Qualifications and Training

Essential

Current entry on the NMC Register/ or professional regulatory health body Professional knowledge to degree level or equivalent, plus diploma level training or equivalent in specialist area and experience Evidence of continuing professional development

Desirable

Evidence of further training/ CPD in learning disabilities and/ or autism In-depth knowledge of Safeguarding Vulnerable Adults and Child Protection and Mental Capacity Act and DoLS / LPS

Experience

Essential

Substantial experience as a band 5 nurse or practitoner Excellent written and verbal communication skills Evidence of passion and interest in learning disability and autism field

Desirable

Current learning disabilities experience in an acute setting Experience of research and audit Experience of handling clinical incidents and complaints
