Job summary

This exciting role is for a Rapid Response & Intermediate Care Clinical Lead (South East) and entails day to day responsibility for the effective and efficient operational and clinical management of the Amersham Rapid Response and Intermediate Care team (RRIC), Southern Rapid Response and Intermediate Care team (RRIC) and Marlow Rapid Response and Intermediate Care team (RRIC). The post holder will work closely and in collaboration with the Rapid Response & Intermediate Care Clinical Lead North and Rapid Response & Intermediate Care Clinical Lead South and UCR to ensure RRIC and the UCR squads operate as one countywide flexible, standardised service across Buckinghamshire.

The post holder will be a highly experienced registered professional, who has the drive and ability to support the transformation and working with their colleagues to ensure that the workforce has the skills and competence that ensures the delivery of the service is safe, responsive and effective in preventing hospital admissions and facilitating hospital discharges.

Main duties of the job

The operational and clinical management includes accountability for budget, workload allocation, capacity management, professional leadership, managing staff performance, clinical governance, development of data and quality information, and direct line management of the Advanced Community Practitioners within the Amersham, Southern and Marlow RRIC teams. They will cover the teams of their counterparts (Rapid Response & Intermediate Care Clinical Lead North and Rapid Response & Intermediate Care Clinical Lead South and UCR) as and when required

EDUCATION, QUALIFICATIONS & TRAINING

Essential

Registered allied healthcare professional or nurse Degree/ masters level qualification relevant to community practice Evidence of recent CPD

Desirable

Post-graduate management and/or leadership training Post Graduate masters level modular training

EXPERIENCE

Essential

Demonstrate broad experience post registration working in a community setting within a variety of core areas In depth experience of working within multi agency / multi-disciplinary team Experience of implementing change management programmes service development Experience of leadership and management

Desirable

Experience participating in a research or audit project and implementing full audit cycle Experience of working with single assessment process

SKILLS, ABILITIES & KNOWLEDGE

Essential

Highly effective verbal and written communication to facilitate effective working with clinical staff, patients and carers, and work colleagues Ability to analyse and interpret complex and varied information in order to identify issues and develop solutions Effective leadership and management skills to lead and motivate staff Understanding of the importance of staff engagement and good Human Resources practice Ability to demonstrate behaviours and attitudes consistent with Trust values

Knowledge of defined service areas, including current issues in service delivery, patient pathways

SPECIAL CIRCUMSTANCES

Essential

Driving licence and car for travelling between sites and throughout Bucks, as required by the post. Business car insurance.
