Job summary

An exciting opportunity has arisen as part of ongoing service development for a motivated individual with well-developed organisational and communication/interpersonal skills to join our Supportive Care Team coordinating support in the community for individuals with life limiting illness.

You will be responsible for the efficient and effective coordination of responsive, individualised supportive palliative care for patients and those important to them. You will be a key player in ensuring coordinated care is in the right place at the right time for each patient and their family including at the end of a persons life so helping to prevent avoidable admissions and facilitate discharge from hospice or hospital.

Working closely with the team leader and colleagues within our Single Point of Access service you will liaise with both specialist palliative care services within the hospice and external agencies ensuring provision of community/fast track care including District Nurses, Marie Curie, Domiciliary Care Agencies and Continuing Health Care creating bespoke packages designed for each individual. There will also be elements of face to face contacts with patients to support responsive admission avoidance intervention and the review of ongoing care provision.

Main duties of the job

Whilst a minimum of an NVQ Level 3(or equivalent)in Health and Social care is required we are just as interested in your experience coordinating and reviewing service needs to deliver care for individuals with life limiting disease and disability.

We provide this in a number of ways; at the hospice and in peoples own homes across our community of Blackburn, Darwen, Hyndburn and the Ribble Valley.

Job responsibilities

KEY PERFORMANCE AREAS

Develop a detailed understanding of East Lancashire Hospice Community Model, knowing the different optionsand levels of provision available to provide supportive palliative care in our community including that deliveredby other organisations in order to maximise available support for patients and their families

Utilise outstanding organisational skills to develop, implement and maintain robust systems and processesnecessary for the effective coordination of supportive palliative care.

Respond creatively and pro-actively to requests for supportive palliative care following assessment by qualifiedmembers of the clinical team, recognising the individuals preferences and choices, including their preferredplace of care

Demonstrate excellent communication skills to engage with patients and their families, both face to face andby telephone, to plan a suitable package of supportive palliative care in line with the initial clinical assessmentand the persons changing needs and circumstances

Contribute to patient review meetings feeding back on the supportive palliative care provided and its reportedimpact

Maintain the provision of supportive palliative care to a caseload of patients and those important to them,liaising with named work to coordinate support visits, conduct scheduled reviews and provide crisisintervention

Produce rosters for supportive care staff and volunteers and organise allocation of support visits taking intoaccount the needs of patients and their families, location, continuity of care and skill mix

Establish and maintain effective communication with hospice supportive care staff and volunteers to highlightany changes in needs, circumstances or risks and adjust the provision of support accordingly

Provide timely feedback on any changes noted or communicated in a persons condition or circumstancestaking account of the potential need for adjustments in service provision

Utilise electronic clinical records systems including EMIS and PAS, to record support provided, progress andproduce reports on contacts, impact and outcomes to demonstrate the effectiveness of supportive palliativecare provision

Conduct dynamic environmental risk assessment where there is an urgent need for unplanned responsiveintervention for example to support early discharge

Answer calls via the ELH response line, recognising when escalation of the call to a qualified member of theteam is required

Contribute to the recruitment, induction, training and support of volunteers providing supportive palliative carein the community

Contribute to audit and monitoring of the provision of supportive care obtaining feedback from patients andtheir families, staff and volunteers identifying good practice, areas for improvement and potential solutions toproblems in order to improve service provision

Promote the work of the community team and wider hospice services throughout our community
