We are actively seeking an experienced care coordinator for our fast growing award winning client based in the Burnley area.

**The role**

This role will include
- Carrying out care assessment for service users
- Updating and reviewing care documents
- Spot checking staff
- Carrying out service surveys quarterly
- Managing staff performance
- Actively managing and maintaining rotas daily
- Building and maintaining professional relationships with Service users, families and other stakeholders
- Maintaining accurate and up-to-date communication records

You would have full support and assistance from office admin staff, registered CQC manager, access to free HR/ legal assistance on a daily basis to help you with your role.

**Job Requirements**:
Must be flexible to work different shift patterns ( 9am to 5pm - 5 days / 5pm to 9pm - 1 day )

Must be able to work 1-2 weekends in a month.

Exceptional people management and communication skills

Computer literate

Minimum of 12 months Care Coordinator experience in a domiciliary care or similar environment

2 years hands on Domiciliary care experience

Achieved NVQ Level 3 in Health & Social Care

Full UK Driving licence.
