Community Palliative Care Coordinator Role

Job OverviewWe are seeking an experienced Community Palliative Care Coordinator to join our Supportive Care Team. As a key member of our team, you will be responsible for coordinating responsive, individualized supportive palliative care for patients and their families. Your expertise will ensure that our patients receive timely and effective care, preventing avoidable hospital admissions and facilitating smooth discharges from hospice or hospital.
Main Responsibilities:- Develop a comprehensive understanding of our Community Model, identifying available options and levels of provision to maximize support for patients and their families.
- Utilize exceptional organizational skills to create and maintain robust systems and processes for effective coordination of supportive palliative care.
- Respond creatively and proactively to requests for supportive palliative care, recognizing patients' preferences and choices, including their preferred place of care.
- Demonstrate excellent communication skills to engage with patients and their families, planning suitable packages of supportive palliative care in line with initial clinical assessments and changing needs.
- Contribute to patient review meetings, feeding back on provided care and its reported impact.
- Maintain provision of supportive palliative care to a caseload of patients and those important to them, liaising with named workers to coordinate support visits, conduct scheduled reviews, and provide crisis intervention.
To succeed in this role, you will require:
- A minimum of an NVQ Level 3 (or equivalent) in Health and Social Care.
- Experience coordinating and reviewing service needs to deliver care for individuals with life-limiting disease and disability.
- Excellent communication and interpersonal skills, with the ability to deal with conflict and often distressing situations.
- A non-judgmental professional approach to customer care, with a sense of pride and achievement in your work.
